关节镜下治疗膝关节半月板损伤7例分析

摘　　要：目的　探讨关节镜下膝关节半月板损伤治疗方法。方法　自 2 0 0 1年 12月至 2 0 0 3年 5月对 7例膝关节半月板损伤进行关节镜下检查及治疗。 7例中 ,右侧 5例 ,左侧 2例 ;内侧半月板损伤 3例 ,外侧半月板 4例。O Connor关节镜下损伤类型 :斜形撕裂 2例 ,水平撕裂 1例 ,纵形撕裂 1例 ,瓣状撕裂 2例 ,复合撕裂 1例 ;半月板部分切除术 3例 ,半月板次全切除术 4例。结果　 7例均经随访 ,随访时间 12～ 30个月 ,平均 2 0个月。 7例患者均于次日进行股四头肌等长训练 ,第 3天膝关节开始屈伸活动 ,第 7天下地行走 ,2周基本恢复正常膝关节活动功能 ,未发生关节镜器械折断在关节腔现象 ,无骨关节炎表现。结论　关节镜技术治疗膝关节半月板损伤具有损伤小、恢复快等优点。Objective To evaluate the methods of the treation for the knee meniscus injury under arthroscopy.Method From December 2001 to May 2003,seven patients with their knee meniscus injuries were examinated and treated under arthoscopic.There were five males and two females.The average age was 41.3 years(ranged from 18 to 73 years).The lesions were right knees in five cases and left knees in two cases.There were medial meniscus in three cases and lateral meniscus in four cases,O'Connor arthroscopic classification there were oblique tears in two cases,horizontal tears in one cases,longitudinal tears in one cases,flap tears in two cases and complex tears in one cases.Localmeniscectomy in three cases,subtotal-meniscectomy in four cases.Results All the patients were arailable at follow-up,was 20 months,ranging from 12 to 30 months.The second day,all of the patients started to contract their quadriceps-muscle on isometric,the third day all of the them started to extensively and flexibly their knees.The seventh day all of the patients started to weightbear and walk.After two weeks their knee was normal.The inflammatory of the knee is didn't appear,the arthroscopy equipment had not be brakted into the intra-articalor.Conclusion For the treatment of the knee meniscus injury,the arthroscopy technique has many advantages,such as miniinvasion and rapid recovery.
