心肌长轴运动位移参数对心梗患者左室局域功能的影响

摘　　要：目的 :探讨心梗病人左心室不同节段心肌沿长轴运动的位移特征及其临床意义 ,寻求评价局域心肌功能的合理量化指标。方法 :利用定量组织速度成像中的速度时间积分曲线对 15例心肌梗死病人 (MI组 )和 16例正常人 (正常对照组 )左室各相关节段长轴方向的位移进行测量分析。结果 :MI组病人收缩期位移峰值明显降低 ,收缩期位移上升速率减低 ,等容收缩期位移峰值减低 ,部分节段出现位移方向的改变 ,等容舒张期梗死节段及对侧壁出现第二次位移峰值。结论 :( 1)心肌缺血导致局域心肌沿长轴方向的位移异常 ;( 2 )心肌沿长轴方向的位移特征可直接反映心肌的运动特征 ,可作为反映左室心肌长轴功能的量化指标。Objective:To detect the regional disp lacement features of left ventricular wall in patients with myocardial infarctio n,to analyze their clinical significance,and to evaluate the reasonable parame ters to estimate regional function. Method:15 patients with myocardial infarction (MI) and 16 norma l subjects underwent integral of velocity-time study along left ventricular lo ng axis.Results:The peak value and ascend speed of velocity in systolic phase of infarcted segment is decreased dramatically.The peak value of veloci ty in isovolumic contraction of infarcted segment is also decreased,some segme nts even appeared the change of displacement direction.The second peak values o f displacement occurred in the infarcted segments and neighbor segments in isovo lumic relaxation.Conclusion:(1) Regional infarction can lead to regional abnorma lities of displacement along long axis.(2)The character of displacement along l ong axis can reflect myocardial systolic and diastolic abnormalities directly,a nd can be a quantitative index in regional myocardial infarction.
