医务人员严重急性呼吸综合征患者检验随访分析

摘　　要：目的　了解在严重急性呼吸综合征 (SARS)流行期及恢复期 ,我院 77例院内感染医务人员血样检测的监测情况。方法　血常规采用 5分类计数 ,淋巴细胞亚群用流式细胞仪法 ,2 0项生化标志使用全自动生化分析仪 ,抗SARS IgG采用EIA法 ,对患者随访 5次 ,在 3个月时有 12 0例正常人对照 ,统计使用方差分析。结果　白细胞及淋巴细胞早期下降。淋巴细胞亚群在病后 3个月大多恢复 ,虽有一定异常率 ,但无统计学意义。生化标志ALT、AST、LDH、γ GT在发病早期均有明显升高 ,在 3个月后复常并稳定。血糖早期升高 ,1 5个月恢复正常。抗SARS IgG在追访的 5个时段内平均 3个月 10 0 %阳性至 12个月仍稳定 ,对照组 1 6 7%阳性。结论　SARS患者的血细胞总数及淋巴细胞绝对计数在疾病早期有辅助诊断价值。患者病后 3个月的NK Lym(CD16 + CD5 6 + )、B Lym(CD19+ )、T Lym(CD3+ )、T Lym(CD3+ CD4 + )、T Lym(CD3+ CD8+ )均无统计学意义改变。疾病早期可能因治疗影响糖代谢 ,并有一定的肝损害。患者抗SARS IgG在发病 3个月时基本全部出现至12个月仍稳定。Objective To analyze the laboratory results of 77 medical staff with severe acute respiratory syndrome(SARS) from hospital infection Methods Blood routine test was done by STKS, Lymphocyte subtypes by cytometry, 20 biochemical markers by automatic biochemistry analyzer, Anti SARS IgG by EIA The patients were followed up 5 times In the 3rd month, 120 normal medical staff were analyzed as control One way ANOVA was used as Statistic method Results During early period of SARS the counts of WBC and Lymphocyte were significantly different compared with control group In the third month there were abnormal distribution of Lymphocyte subtypes were observed in some patients Some biochemistry markers(ALT、AST、LDH、γ GT) increased absolutely during early period of SARS, recovered in the 3rd month and were stable in the 12th month Blood glucose increased during early period of SARS Anti SARS IgG was positive since the 3rd month and still stable in the 12th month In control group, only 1 67 percent was positive Conclusion WBC count and lymphocyte count were helpful for diagnose of SARS in the early stage After 3 months since suffering from SARS, lymphocyte subtypes count was of no statistics significance In early period of SARS, glucose metabolic was affected and Liver was damaged In all patients, the anti SARS IgG were found from the 3rd month with SARS and stable until the 12th month
