急性有机磷农药中毒患者血液流变学指标变化的意义

摘　　要：目的探讨急性有机磷农药中毒(AOPP)患者血液流变学指标改变的临床意义。方法选择了30例急性有机磷农药中毒患者,按入院时中毒程度分为轻度中毒组和中、重度中毒两组,在入院第1天和第7天分别采血进行血液流变学指标的检测(其中包括全血高切黏度、全血低切黏度、血浆黏度、血浆纤维蛋白原、血细胞比容及红细胞沉降率),与健康对照组比较,中毒相同时间两个中毒组间的比较,中毒组内第1天和第7天比较。结果中毒第1天,中毒组与健康对照组比较,各项均明显增高,差异显著(P<005和P<001)。中毒第7天,中毒组与健康对照组比较,轻度中毒组基本恢复正常(P>005),而中、重度中毒组恢复缓慢,6项中仍有3项显著增高(P<005);两个中毒组间比较,中、重度中毒组仍有5项高于轻度中毒组,3项差异显著(P<005)。结论AOPP患者血液流变学指标的改变与中毒程度有关,动态观察血液流变学指标的改变,对判定中毒程度和指导治疗有重要意义。Objective To study the clinical meaning of hemorheology in the patients with acute organic phosphorus intoxication.Methods 30 cases with acute organic phosphorus intoxication according to the poisoning degree were divided into two groups: slight group, moderate and severe group. Blood of the cases were drew on the 1st and 7th day after admitting into hospital in order to examine the hemorheological indexes including η_H,η_L,blood viscosity,plasma fibrinogen, hematocrit(Hct), erythrocyte sedimentation rate(ESR). ①two poisoning groups compared with the healthy group. ②the two poisoning groups compared with each other.③the 1st day's indexes compared with the seventh day's indexes in the poisoning groups.Results Compared with the healthy group, on the first day all the indexes of the poisoning groups were obviously higher .There were significant difference (P<0.05,P<0.01); on the seventh day, slight group recovered basically, but 3 indexes in moderate and severe group increased remarkably(P<0.05).5 indexes were higher in moderate and severe group than in slight group, there were manifest difference in 3 indexes(P<0.05).Conclusion Changes of hemorheological indexes in acute organic phosphorus intoxication cases were closely associated with poisoning degree. Dynamic observation of hemorheological indexes was important to guide treatment and judge poisoning degree. [
