先天性心脏病心内直视手术术后心电图分析

摘　　要：目的比较心脏停跳和不停跳心内直视手术方法对先天性心脏病患者术后心电图变化的影响.方法分析我科2003-10～2004-04接受心内直视手术的358例先天性心脏病患者从手术中至手术后2 d的心电图变化.其中心脏停跳组159例,心脏不停跳组199例.结果两组间比较,心脏停跳组房早、室早、交界性早搏、传导阻滞发生率及术后酸中毒、低心排综合征、高血钾发生率均高于心脏不停跳组.结论心脏不停跳先天性心脏病心内直视手术心律失常发生率低于心脏停跳下施行手术,且手术中易于观察心电图改变,能够及时发现房室传导阻滞发生,便于及时处理.Objective To evaluate the changes of electrocardiogram (ECG)during and after intracardiac procedures in the patients with congenital heart disease(CHD).Methods We analyzed the data of ECG of consecutive 358 patients with CHD,from October 2003 to April 2004,underwent intracardiac procedures in our department.358 patients with CHD were assigned to beating-heart group (n=199)and arrested-heart group (n=159).Electrocardiogram were observed from cardiopulmonary bypass(CPB)to 2 days after operation.Results Atrial fibrillation,ventricular fibrillation,atrial-ventricular block,acidosis,low output syndrome,low potassium in arrested-heart group were apparently higher than those in beating-heart group.Conclusions Arrhythmia in the beating-heart group is lower than that in arrested-heart group.Atrial-ventricular block can be found easily during intracardiac procedure, so the surgeon can deal with it in time.
