原发性肺动脉高压患者的运动心肺功能(附二例报告)

摘　　要：报告2例经心导管检查证实为原发性肺动脉高压的女性患者运动心肺功能改变特点。其PA及mPA分别为10/4.7kPa、6.5kPa及9.7/3.6kPa、5.3kPa。1.(?)/(?)比率失调、气体交换异常,表现为低氧血症,P(A-a)O_2异常增宽,VD/VT增加以及P(a-et)CO_2为正值;2.心输量减低导致(?)O_2max、AT及(VO_2/HR)max减低;3.通气储备BRmax及DImax正常,但于AT时的二氧化碳通气当量(?)E/(?)CO_2 AT明显增高。Cardiopulmonary exercise testing (CPX) in two cases with primary pulmonary hypertension (PPH), proved by catheterization, were reported. PA and mPA were 10/4.7 kPa, 6.5 kPa and 9.7/3.6 kPa, 5.3 kPa respectively. The characteristics of CPX were showed as follows:(1) mismatching V/Q and abnormal gases exchange, hypoxemia and a widened P(A-a)O_2, high VD/VT and positive P(a-et)CO_2; (2) decreased cardiac output resulted in lowering of VO_2 max, AT and (VO_2/HR)max; (3) normal ventilatory reserve i.e. BRmax and DImax but (VE/VCO_2)AT elevated significantly.
