摘 要:目的 探讨乳果糖对烫伤大鼠肠道组织中白细胞介素 18mRNA (IL 18mRNA)表达的影响。方法 将大鼠致30 %体表面积Ⅲ度烫伤 ,随机分成烫伤对照组 (C组 )及乳果糖预防组 (L组 )。采用逆转录PCR技术检测肠组织中IL - 18mR NA水平 ;以分光光度法检测小肠组织二胺氧化酶 (DAO)活性 ;取回盲部肠系膜淋巴结 (MLN)及全血检测细菌含量。结果 伤后C组肠组织中IL 18mRNA表达较伤前增强 (P <0 .0 1) ,并显著高于L组 (P <0 .0 5 ) ;L组在伤后 8、16、2 4h小肠DAO活性高于C组 (P <0 .0 5 ) ,但L组伤后 2 4h内血培养的细菌阳性率及MLN细菌数量均低于C组 (P <0 .0 5 )。结论 烧伤早期肠道IL 18mRNA表达明显过度 ,致肠黏膜屏障遭受损害 ,细菌 /内毒素移位增加 ,而预先应用乳果糖可在一定程度上保护肠黏膜屏障 ,减少细菌 /内毒素移位。Objective To investigate the effect of oral lactulose on interleukin-18 mRNA expression in severe burned rats.Methods The rats were subjected to full-thickness thermal injury on 30% of total body surface area. They were randomly divided into two groups:scald controls (Group C) and lactulose group (Group L). The level of IL-18 mRNA in tissues of intestine was determined by reverse transcription polymerase chain reaction.Diamine oxidase (DAO) activity of small intestine was assayed by chromatography. The tissues of mesenteric lymph nodes (MLN) in ileocecum and whole blood were cultured at 2,8,16 and 24 hours after burn.Results The IL-18 mRNA expression in group C was significantly higher than that in group L after burn injury (P< 0.05). DAO activity in group L was significantly higher than that in group C at 8,16 and 24 hours after burn, while the positive rate of blood bacterial culture and bacterial count in MLN after burn injury were lower in group L than in group C (P<0.05).Conclusion IL-18mRNA over expression in intestine results in the damage of intestinal barrier and the high incidence of bacterial/lipopolysaccharide(LPS) translocation to MLN and whole blood in early stage of burn.The lactulose therapy protects intestinal barrier from high incidence of bacterial/LPS translocation.
