全营养混合液静脉营养在极低出生体重儿应用的效果观察

摘　　要：目的探讨全营养混合液静脉营养对极低出生体重儿(VLBWI)体重及并发症的影响。方法将16例给予全营养混合液静脉营养的VLBWI与18例未给予静脉营养的VLBWI进行对照研究,记录患儿体重下降程度、体重开始增加的日龄、第21天前每天增加体重及并发症发生等情况,并将资料进行统计学分析。结果治疗组体重下降208±120g,出生后5.2±2.6d体重开始增加,每天体重增加21.6±8.8g;对照组体重下降316±136g,出生后10.3±5.8d体重开始增加,每天体重增加11.5±6.4g。两组比较,均P<0.05,差异均有统计学意义。治疗组1周内有43.8%患儿发生高血糖,25.0%出现低血糖症状,无1例出现体位性水肿;对照组1周内有83.3%患儿发生高血糖,33.3%出现低血糖症状,有12例出现体位性水肿。两组比较,均P<0.05,差异均有统计学意义。结论全营养混合液静脉营养可明显提高VLBWI体重增加的速度及幅度,减少并发症的发生,是VLBWI综合治疗中安全而又必不可少的一项措施。Objective To study the effects of all,in,one parenteral nutrition on very,low,birth,weight infants (VLBWI). Methods A retrospective case,control study of 34 VLBWI was conducted. Sixteen VLBWI in the study group received all,in,one parenteral nutrition. Eighteen VLBWI in the control group did not receive all,in,one parenteral nutrition. The clinical data were analyzed retrospectively. Results In the study group, the newborns average weight was 208±120 g at birth, and the weight began to increase at day 5.2±2.6 after birth with an increase of 21.6±8.8 g per day. In the control group, the newborn's average weight was 316±136 g at birth, and the weight began to increase at day 10.3±5.8 after birth with an increase of 11.5±6.4 g per day. There was significant difference between the two groups (P < 0.05). In the study group, hyperglycemia was found in 43.8% of infants within the first week. In the control group, hyperglycemia was found in 83.3% of infants within the first week. There was significant difference between the two groups (P < 0.05). Conclusions All,in,one parenteral nutrition can significantly accelerate the speed of weight increase and decrease the occurrence of complications. It is a safe and absolutely necessary measure to the therapy of VLBWI.[
