膀胱镜下尿道会师术治疗体会(附26例报告)

摘　　要：目的探讨膀胱镜在尿道断裂会师术中的应用效果.方法对26例男性尿道球部或膜部断裂病人在膀胱镜下进行尿道会师术.结果26例均获得成功,术后随访6个月～2年,15例无继发尿道狭窄,7例拔尿管2周后出现尿道狭窄,经尿道扩张3个月后缓解,3例拔尿管1周后出现尿线变细,经尿道扩张半年后缓解,1例骨盆严重畸形,拔尿管5 d后出现尿线变细,每周均需行尿道扩张,3个月后仍无缓解,改开放手术.结论膀胱镜下行尿道会师术创伤小,达到一期恢复尿道连续性,恢复快,是治疗尿道断裂有效的微创手术方法之一.Objective: To inquire into the effect of the urethral reunion operation by cystoscope. Methods: To perform urethral reunion operation by cystoscope on twenty six male patients whose membranous urethra or bulbus urethra were broken. Results: All cases were successful. The postoperative follow-up after six months to two years showed, fifteen cases had no urethrostenosis, seven case had it after removing the urethral catheter, for two weeks, and remitted after three months by urethral dilatation. The urine streams of three cases became smaller, and remitted after half a year by urethral dilatation. One had severe deformity of pelvis, the urine stream became smaller after removing the urethral catheter for five days, he had to urdergo urethral dilatation every week, but had not remitted after three months, and had to undergo open operation. Conclusion: The initation is slight, recover the urethral continuity quickly, is one of the effective mircoinjured operation to the therapy of urethral rupture.
