摘 要:目的观察健康婴儿在不同喂养方式下肠道菌群的定植过程及其在食物过敏时的变化,分析肠道菌群形成与婴幼儿食物过敏的相互关系及母乳喂养的意义。方法采用荧光定量PCR技术测定细菌16SrRNA,对健康无过敏症131例(其中母乳喂养71例,人工喂养60例)及100例过敏症婴儿粪便中乳酸杆菌、双歧杆菌和大肠杆菌进行定量检测。结果婴儿肠道菌群处于动态定植过程。母乳喂养婴儿肠道乳酸杆菌、双歧杆菌数量较人工喂养婴儿高(P<0.05),而大肠杆菌数量较人工喂养婴儿低(P<0.05)。食物过敏婴幼儿肠道乳酸杆菌、双歧杆菌数量较健康婴幼儿低(P<0.05),而大肠杆菌数量较健康婴幼儿高(P<0.05)。结论婴儿期肠道菌群处于动态演替过程。不同的喂养方式对肠道菌群有影响,过敏性疾病婴儿肠道菌群与健康婴幼儿不同,应大力提倡母乳喂养。To test the hypothesis that food allergy among infants may be associated with differences in their feeding fashion and investigate the relationship between formation of intestinal microflora and food allergy in infants, as well as elucidate the significance of breast-feeding. Methods 71 healthy breast-fed,60 formula-fed healthy infants and 100 allergic infants were studied. Real-Time PCR procedures based on 16S rRNA gene sequences specific for Bifidobacerium,Lactobacillus and Escherichia coli that predominate in the human intestinal tract were developed and used for quantitative detection of these species in infants.Results After birth there were Bifidobacteria colonized in infantile intestine, then their numbers increased rapidly and reached 5 times at 6-month age,which became the preponderant flora. Lactobacilli were also presents in infantile intestine one month after birth and augment gradually. The number of Escherichia coli was less than Bifidobacteria and Lactobacilli and appeared to decline during the early infants.Bifidobacteria were the predominant bacteria in breast-fed infants P<0.05 in comparison with the formula-fed infants.Similarly,in breast-fed infants,the proportions of Lactobacilli were higher than those in the formula-fed.In contrast,the formula-fed infants harbored higher counts of Escherichia coli P<0.05. The number of Bifidobacteria and Lactobacilli in the food allergic infants were much less than that in the healthy infants P<0.05,but Escherichia coli was significantly higher than that in the healthy infants P<0.05,in comparison with the nonallergic infants. Conclusions During the first year of life the intestinal microflora was in a developing proceeding,compared with the healthy infants,Bifidobacteria and Lactobacilli were decreased,but Escherichia coli was increased in the allergic infants.This study confirms the differences in development of intestinal flora between breast-fed and formula-fed infantsindicating that the probiotics may be benefit to the p
