超声治疗前后外阴白色病变的超微形态比较研究

摘　　要：目的:了解超声治疗对外阴白色病变患处上皮细胞及真皮组织超微结构形态的影响。方法:30例患者分别做治疗前及治疗后3～6月的外阴皮肤组织活检,用光镜和透射电镜观察。结果:治疗前,表皮显示不同程度的上皮增生或萎缩并伴过度角化;细胞间隙增大,桥粒减少;细胞内黑色素颗粒减少,线粒体内室肿胀和局部胞质溶解;黑色素细胞少见;真皮内毛细血管减少,管腔多呈皱缩状;表皮及真皮内常见淋巴细胞浸润。治疗后,病变皮肤组织呈不同程度恢复,表皮细胞排列整齐,桥粒及胞质内黑色素颗粒均有不同程度增加,基底层常见黑色素细胞;真皮内成纤维细胞多见,毛细血管增多且管腔形态正常;浸润的淋巴细胞明显减少。结论:超声治疗可使患处上皮细胞及真皮组织的超微结构和色素代谢趋于恢复正常。Objective:To investigate ultrastructural effect of ultrasonic treatment on the epithelial cells and dermis tissues of white lesions of the vulva. Methods:A total of 60 patients with white lesions of the vulva were treated by ultrasonic treatment. The biopsy specimens obtained from prior to therapy as well as 3～6 months after ultrasonic treatment in 30 patients were studied by transmission electron microscope combined with light microscope.Results:Before ultrasonic treatment, the epidermis showed various degree of hyperplastic or atrophic changes including a very thicking of epidermal keratinization and an enlargement on cellular space of stratum spinosum as well as decrease of desmosomes. At the same time, a decrease in number of melanotic granules and swelling of mitochondria in basal cells and spinous cells were observed. In addition, a few contractive capillaries and some infiltrative lymphocytes were noted also. After ultrasonic treatment, however, both epidermis and dermis showed a sign of resuming normal pattern at different degrees. The stratified squamous epithelium of vulva recovered normal stratification and thickness. The quantity of desmosomes and melanotic granules was increased, and melanocyte could be seen obviously in the basal layer of epidermis. Most of capillaries in dermis showed a normal form. Conclusions:The results suggested that ultrasonic treatment could mitigate effectively ultra-pathologic changes of the epidermis and dermis, and promote degenerative tissues of white lesions of the vulva to resume to a normal pattern.
