股骨头缺血性坏死的手术选择及疗效分析

摘　　要：作者对1977～1990年间的78例股骨头缺血性坏死手术治疗进行了分析。手术包括闭孔神经、内收肌切断术8例;粗隆下截骨术10例;股骨颈钻孔减压、血管束植入或肌骨瓣移植术44例(47个关节),其中儿童4例;髋关节成形术13例;髋融合术3例。78例平均随访时间5.7年。结果表明:优、良者61例,占78.2％。对该病的手术选择、术后效果、早期诊断进行了讨论。This paper presents a report of 78 patients with avascular necrosis of the femoral head (ANFH) who underwent operation in the period 1977 to 1990, of whom, 8 received obturator neurotomy and adductor tenotomy; 10 subtrochanteric osteotomy; 44 (47 hips) femoral head-neck drilling musclepedicle bone grafting or vesselpedicle grafting 13 arthroplasty; and 3 hips arthrodesis. 78 cases were followed up for an average of 5. 7 years. Surgical results show that drilling and muscle-pedicle or vessel-pedicle grafting proves to be more effective than other therapies with 2 success rate of 78. 2% (61 cases). Selection of operation, postoperative effects and early diagnosis are also discussed.
