护理相关因素对CT增强肺动脉造影诊断亚段肺动脉栓塞的影响

摘　　要：目的探讨在肺栓塞(PE)CT增强肺动脉造影检查中影响亚段肺动脉栓塞诊断的几种护理因素,优化护理配合方案,提高诊断水平。方法回顾性分析行CT增强肺动脉造影检查和ECT肺通气/灌注扫描的PE患者32例,根据造影剂注射部位的不同将患者随机分为A组(经右肘静脉)和B组(经左肘静脉)各16例,观察两组患者造影剂在肺动脉的充盈状况;根据患者的心率(100/min以上或以下)及呼吸(20/min以上或以下)各分成两组,观察心率和呼吸频率对血管显示清晰度的影响。结果同一层面、同一时相A组造影剂在亚段肺动脉显示清晰度优于B组;心率和呼吸频率对血管显示清晰度有较大影响。结论注射造影剂以穿刺右侧肘静脉为优;恰当的心理护理,适度调节心率和呼吸频率可明显提高亚段PE的检出率,是保证检查顺利完成的关键。Objective To explore nursing-related factors influencing the diagnosis of subsegmental pulmonary embolism ( SPE) on contrast-enhanced CT pulmonary angiography so as to optimize nursing programs and raise diagnostic level of SPE. Methods 32 patients diagnosed with PE were retrospectively analysed. All patients underwent contrast-enhanced CT pulmonary angiography and ventilation-perfusion ( V-P) scin-tigraphy to observe the filling state of the contrast media in the pulmonary arteries. The patients were e-qually randomized into two groups: group A where injection of the contrast medium was through the vein of the right arm, and group B where injection was through the vein of the left arm to observe the influence of heart rate and respiratory frequency on the display quality of the pulmonary arteries. Results In the same section and time phase, group A excelled group B in display quality of the subsegmental pulmonary arteries so far as filling of the contrast medium was concerned. Heart rate and respiratory frequency had significant influence on the outcome of angiography. Conclusion The right elbow vein is the best choice of puncture for enhanced CT pulmonary angiography. Appropriate psychological nursing and proper adjustment of heart rate and respiratory frequency are key factors of improving the yield rate of PE in subsegments, and of completing the procedure uneventfully.
