胸骨劈开对心肺转流心脏手术患者血浆炎性反应介质的影响

摘  要:目的比较胸骨劈开前后心肺转流(CPB)瓣膜置换术患者P-选择素(CD62P)、白细胞介素-8(IL-8)和白细胞介素-10(IL-10)血浆水平变化.方法 12例择期行CPB心瓣膜置换手术的患者,ASA分级Ⅲ~Ⅳ级,分别于麻醉前和胸骨劈开后15 min,采用酶联免疫法定量测定P-选择素的血浆水平、采用放射免疫方法定量测定IL-8和IL-10血浆水平.结果血浆CD62P、IL-8和IL-10麻醉前和胸骨劈开后相比差异无显著性(P>0.05). 结论 CPB心脏手术时胸骨劈开这一强烈手术刺激不足以引起血浆炎性反应介质的大量释放.Objective To compare the plasma differences of P selectin, interleukin 8 (IL 8) and interleukin 10 (IL 10) pro and after sternotomy in patients undergoing replacement of cardiac valve with cardiopulmonary bypass (CPB). Methods Twelve ASA Ⅲ~Ⅳ adult patients undergoing elective cardiac valve replacement with CPB were enrolled to the study. Blood was drawn before anesthesia and 15 min after sternotomy from central vein respectively. Concentrations of P selectin(CD62P) were measured using a quantitative sandwich enzyme linked immunosorbent assay (ELISA). The concentration of IL 8 and IL 10 on blood plasma was measured by radioimmunoassay (RIA). Results There were no significant differences of plasma concentration of P selectin, IL 8 and IL 10 in central vein on the sample point 15 min after sternotomy compared with that before anesthesia respectively. Conclusion The sternotomy may do not influence the inflammatory response in patients undergoing replacement of cardiac valve with CPB.
