摘 要:目的 探讨中国人血清脂联素水平与肥胖及肥胖类型的关系。方法 对 390名居住上海地区中国人应用MRI测定局部体脂,放射免疫分析法测定血清脂联素水平。结果 ( 1 )女性血清脂联素水平显著高于男性(P<0. 01)。(2)超重 /肥胖者血清脂联素水平较正常体重者显著下降〔男性 (8. 59±0. 64)mg/Lvs (10. 73±0. 95)mg/L(P<0. 05); 女性 (11. 42±0. 58)mg/Lvs (13. 59±0. 84)mg/L(P<0. 05)〕,腹内型肥胖者血清脂联素水平较非腹内型肥胖者显著下降〔男性(8. 32±0. 82)mg/Lvs(9. 58±0. 99)mg/L(P<0. 05);女性(10. 74±0. 58)mg/Lvs(12. 16±0. 88)mg/L(P<0. 05)〕。(3)多元逐步回归表明,无论性别,腰臀比、腹部脂肪面积 /股部脂肪面积 (VA+SA/FA)、低密度脂蛋白胆固醇、HOMA IR是血清脂联素水平的独立相关因素,表现为两者之间的负性影响。结论 血清脂联素水平存在性别差异;腹内型肥胖者伴有显著的低脂联素血症;血清脂联素与腹内型肥胖、胰岛素抵抗指数显著负相关。Objective To study the association of serum adiponectin concentrations with the types of obesity. Methods A total of 390 subjects, residents of Shanghai, participated in the study. Fasting serum adiponectin was assayed by RIA and regional body fat was measured by MRI. Results (1) Serum adiponectin concentration in women was significantly higher than that of men (P<0.01). (2) As compared with normal weight subjects, the serum adiponectin concentration decreased significantly in overweight/obese subjects 〔(8.59±0.64)mg/L vs (10.73±0.95)mg/L in men ( P<0.05) and (11.42±0.58)mg/L vs (13.59±0.84)mg/L in women (P<0.05)〕; moreover, it decreased more obviously in subjects with visceral obesity than those with non visceral obesity〔(8.32±0.82)mg/L vs (9.58±0.99)mg/L in men (P<0.05) and (10.74±0.58)mg/L vs (12.16±0.88)mg/L in women ( P<0.05)〕. (3) Multiple regression analysis showed that waist hip ratio, visceral adiposc area plus abdominal subcutaneous adipose area/femoral subcutaneous adipose area, low density lipoprotein cholesterol, HOMA IR were the main independent relative factors influencing serum adiponectin concentration in both men and women. Conclusion Serum adiponectin concentration is different between men and women. Visceral obesity is associated with hypoadiponectinemia. Serum adiponectin concentration is negatively correlated with visceral obesity and the degree of insulin resistance.
