摘 要:目的 了解严重急性呼吸综合征(severe acute respiratory syndrome,SARS)患者临床特征,中医证候演变规律,探讨舌象特点与血象变化之间的关系及舌象在防治 SARS中的作用。方法 采用前瞻性方法对入院的214例次SARS患者的临床表现、舌象照片、血象检查等临床资料归纳总结,统计分析。结果 患者白细胞计数(WBC)、中性粒细胞比例(N %)与舌苔腐腻成极显著正相关(P<0. 01);淋巴细胞比例(L %)、嗜酸细胞比例(E %)、单核细胞比例(M %)与腐腻成极显著负相关(P<0. 01);E %与舌芒剌、裂纹、舌尖红成极显著正相关(P<0 .01);M %与老嫩、舌红、润燥、剥脱成极显著正相关(P<0 .01),与舌淡红成极显著负相关(P<0. 01);血小板计数与舌体瘦胖、裂纹成显著正相关(P<0. 05),与舌红、苔厚薄成显著负相关(P<0. 05)。患者的舌象与其年龄和性别无显著关系。结论 SARS患者的临床表现,舌象、血象有独特改变,且舌象与血象之间具有一定的相关性,舌象的动态观察有助于及早提示病情变化,中医舌诊在SARS诊疗中具有重要意义。Objective To find out clinical characteristics of the symptoms and regularity of pathological development of SARS according to TCM syndrome differentiation, and to investigate the role of tongue patterns in preventing and treating SARS as well as the relationship between tongue patterns and changes in the blood pictures in the SARS patients. Method A prospective approach was adopted to generalize and statistically analyze the clinical manifestations, photos of tongue patterns, and results of the examination of the blood pictures in 214 SARS inpatients. Results It was found that there was a marked positive correlation between SARS patient white blood cell count and neutrophil percentage, and the condition of the tongue fur, such as curdiness and greasiness (P<0.01); there was a marked negative correlation between lymphocyte percentage, eosinophil percentage and monocyte percentage, and the condition of the tongue fur, such as curdiness and greasiness (P<0.01); there was a marked positive correlation between eosinophil percentage and the condition of the tongue proper, such as prickliness, flawing and red tip (P<0.01); there was a marked positive correlation between monocyte percentage and the condition of the tongue proper, such as toughness, tenderness, redness, moisture, dryness and denudation (P<0.01); there was a marked negative correlation between monocyte percentage and pale-redness of the tongue proper (P<0.01); there was a marked positive correlation between blood platelet count and the condition of the tongue proper, such as shrinking, enlargement, and flawing (P<0.05); and there was a marked negative correlation between blood platelet count, and redness of the tongue proper and thickness and thinness of the tongue fur (P<0.05). It was also found that there was no obvious relationship between the patients' tongue patterns, and ages and sexes.
