摘 要:目的探讨Parkland公式对犬海水浸泡弹烧复合伤血液流变学的影响。方法成功建立海水浸泡弹烧复合伤模型后 ,依模型将14只成年杂种犬随机分为对照组 (即海水浸泡弹烧复合伤组,n=8)和标准组(标准Parkland公式复苏组 ,n=6) ,动态监测伤前、伤后4、7、10、20、28h共6个时相点血液流变性变化。结果①两组动物血浆黏度 (PV)与伤前比较变化均不明显;②标准组不能纠正全血高切黏度 (HBV)及红细胞聚集指数 (AI)浸泡后较伤前恶化的情况 ,两组之间无明显差异 ;③标准组对于改善全血低切黏度 (LBV)、血细胞比容 (Hct)、红细胞变形指数 (DI)有一定价值。结论Parkland公式复苏对改善海水浸泡弹烧复合伤犬血液流变学特性有一定效果 ,但仍存在不足 ,需要进一步改进。Objective To explore hemorheologic effects of resuscitation with Parkland formula on projecˉtile_burn combined wound in dogs with seawater immersion.Methods Create a model of projectile_burn combined wound in dogs with seawater immersion,fourteen dogs were randomized into two groups:projectile_burn combined wound with seawater immersion(immersion group),Parkland formula resuscitation(resuscitation group I).Record the hemorheologic changes before injury,4、7、10、20、28h after injury.Results1.There was no significant change of plasma viscosity between two groups compared to pre_wounded value;2.It can not significantly rectify the aggraˉvation of HBV(High shear blood viscosity)and AI(Erythrocyte aggregation index)resuscitation according to Parkˉland Formula.There was no significant change of HBV/AI between two groups compared to pre_wounded value.3.It can significantly rectify the aggravation of LBV(Low shear blood viscosity)、Hct(Haematocrit)and DI(Erythrocyte distortion index)resuscitation according to Parkland Formula.Conclusions Fluid resuscitation according to Parkland Formula can amend the changes of hemorheologics to a certain degree,however,it has defiˉciency and need improving.
