摘 要:目的探讨慢性肺心病肺动脉高压患者中血红素氧合酶(HO)-1和内源性一氧化碳(CO)的变化。方法选择20例心超肺动脉压力高于30mmHg的肺心病患者作为治疗组,在急性发作期治疗前后分别用分光光度法测定HO-1和HbCO,并和正常人群进行比较。结果慢性阻塞性肺疾病(COPD)肺心病患者急性发作期HO-1、HbCO为(0.167±0.032)A、(5.115±0.921)%,病情稳定两周后复查为(0.115±0.046)A、(4.269±1.317)%,两者相比差别有显著性意义穴P<0.01雪。同时病情稳定期为(0.049±0.124)A、(1.211±0.477)%,亦明显高于正常人群穴P<0.001雪。治疗前血液HO-1、HbCO浓度与肺动脉压力呈正相关(r=0.903和0.918熏P<0.001)。结论COPD肺心病患者血HO-1、HbCO明显高于正常人群,并与肺动脉压呈正相关。提示血红素氧合酶蛐一氧化碳体系可能参与了机体对缺氧条件下肺血管重构的自身调节。Objective To study the changing of heme-oxygenase-1(HO-1) and endogenous carbon-monoxide(HbCO) in patients with chronic pulmonary hypertensio n. Methods Blood HbCO levels and HO-1 activity in 20 patients with pulmonary hy pertension and 20 healthy controls were measured, and pulmonary artery pressure was also examined with ultrasonography. Results The activity of HO-1 and HbCO l evels of pulmonary hypertension patients before treatment were (0.167±0.032)A a nd (5.115±0.921)%, significantly higher than those after treatment ( P<0.01).Wh en the patients were stable the activity of HO-1 and HbCO [(0.049±0.124)A and ( 1.211±0.477)%] were still higher than that of normal people (P<0.001). Both HO-1 and HbCO were positively correlated with pulmonary artery pressure. (r=0.903 a nd 0.918, respectively P<0.001). Conclusion The results from this study indicat e that CO/HO-1 system might be involved in the self-regulation of hypoxic pulmon ary blood vessel reconstruction in patients with cor pulmonale.
