针刀治疗臀上神经卡压症的手术入路研究

摘　　要：为研究针刀治疗臀上神经卡压症的手术入路 ,在 3 0侧成年尸体的臀区观测了臀上神经、血管出梨状肌上孔处的横径、纵径及距棘角线的距离及其相互关系 ;梨状肌出盆处的宽度、厚度、上下缘距体表的距离 ,其上下延长线与棘交点至髂后上棘角线的距离。结果测得臀上神经出梨状肌上孔处横径为 3 .98± 1 .0 8mm ,纵径为 1 .3 8± 0 .42mm ,与棘角线的距离 47.0 7± 6.83mm ,测得梨状肌出盆处的宽度为 3 3 .0 3± 3 .0 6mm ,厚度为1 3 .68± 3 .5 7mm。据此讨论了形成臀上神经、血管卡压症的解剖学机制 ,并设计了较合理的针刀治疗此病症的手术入路 ,指出了术中应注意避免损伤的结构。To research the operation pathway of superior gluteal neve compression syndrome by small needle-kinife, thirty antisepsis adult gluteal specimens were studied to observe the diameter of nerve, blood vessel and its relation from the upper hole of the piritormis, and the pelvis.The distance to the line of posterior iliac spine and scaral cornu,the width,thick of piriformis from pelvis.The distance of its upper and lower to the body surface,the intersection of the upper and lower line with the line of the posterior iliac spine to scaral cornu.Conclusion:The transevrse diameter of the upper hole of piritormis was 3.98±1.08?mm,longitudinal diameter was 1.38±0.42?mm the distane to the line of the posterior iliac spine and scaral cornu was 47.07±6.83?mm,the width of piritormis from pelvis was 33.03±306?mm,the thickness was 13.68±3.57?mm.Its anatomic mechanism and reasonable operation pathway were discussed.
