获得性免疫缺陷综合征眼部损害的临床特征

摘　　要：目的　了解获得性免疫缺陷征眼部损害的临床特征。方法　对在贝宁共和国纳迪丹古地区医院眼科接诊的 35例艾滋病患者的眼部损害进行回顾性分析 ,其中 12例患者因为眼部损害为首发症状就诊而获知为艾滋病感染者。结果　 35例患者均有不同程度的眼部损害 :视力减退、眼底检查均有视网膜微血管异常 ;其中 2 / 3的患者眼底可见棉絮斑 ;16例患者诊断为巨细胞病毒性视网膜炎 ,其中 7例为眼部首发症状的艾滋病感染者 ,5例发生不同程度的视网膜脱离 ;8例患者患眼带状疱疹 ,其中 5例为眼部首发症状的爱滋病感染者 ,3例合并全身多处带状疱疹 ;1例患者并发单眼上睑下垂、复视、眼外直肌麻痹 ;3例患者并发眼睑、结膜卡氏肉瘤。结论　 75 %以上的艾滋病患者眼部会受到侵犯 ,有些眼部损害为艾滋病的首发症状。眼科检查对艾滋病的诊断、治疗。Objective To find out the clinical features of acquired immunodeficiency syndrome about eye injuries.Methods Reviewing and analyzeing the eye injuries of 35 AIDS patients who were examined by Eye Hospital in Natitingou, the Republic of Benin, of which 12 patients were found as AIDS through the examination of first symptom about eye injuries.Results 35 patients are all with eye injuries differently: the eyesight fails and the micro-vessel in retina is abnormal through the eyeground examination; 2/3 patients have cotton woolspot in the eyeground; 16 patients are diagnosed as cytomegalovirus retinitis, of which 7 patients with AIDS are the first symptom in eyes; 5 patients have retinal detachment in a certain degree; 8 patients have herpes zoster in eyes, of which 5 patients with AIDS are the first symptom in eyes, 3 patients have many herpes zosters from head to foot; 1 patient has the complication of ptosis in one eye, diplopia, palalysis of rectus eye muscle; 3 patients have the complication of Kaposi's sarcoma in eyelid, conjunctiva.Conclusion Above 75% AIDS patients will be injured in eyes, and some eye injuries are the first symptom of AIDS. The eye examination is very important to the diagnosis, treatment and prevention of AIDS.
