^(99)Tc^m-DTPA肾动态显像对静脉肾盂造影不显影患肾功能评价的临床研究

摘　　要：目的　探讨放射性核素99Tcm-DTPA肾动态显像对静脉肾盂造影不显影患肾功能评价的临床意义。方法　对 2 3例静脉肾盂造影不显影的患肾进行99Tcm-DTPA肾动态显像结合临床进行分析。结果　患肾99Tcm-DTPA肾动态显像主要表现为 2 - 3级 ,即放射性摄取中度以上减少 ,占 6 5 .2 %。其中Ⅰ级 2例占 8.7% ,Ⅱ级 5例占 2 1.7% ,Ⅲ级 10例占 4 3.5 % ,Ⅳ级 6例占 2 6 .1%。GFR均值分别为 33.5ml/min、2 5 .6ml/min、16 .8ml/min、8.9ml/min ,无 0级。手术 2 2例 ,切除 18例 ,保留 4例 ,保留患肾功能恢复良好。结论　99Tcm-DTPA肾动态显像对帮助术前判断患肾是否还有保留价值具有重要的临床意义。此法具有简便 ,无创伤 ,灵敏度高及可重复性的特点 。Objective To investigate the clinical cignificance of 99Tc m-DTPA renal dynamic imaging on evaluate renal function as IVP detection failed.Methods 23 patients which IVP detection failed were accepted 99Tc m-DTPA renal dynamic imaging was.Results 99Tc m-DTPA renal dynamic imaging was shown that gradeⅠ was 2 cases,Ⅱwas 5 cases,Ⅲ was 10 cases,Ⅳ was 6 cases. Average GFR were 33.5 ml/min,25.6 ml/min,16.8 ml/min and 8.9 ml/min respectively. 22 cases were operated,including 18 cases nephrectomy, 4 cases kidney reserved.Conclusion 99Tc m-DTPA is an ideal methods to estimate preoperative renal function for IVP detection failed patients. It is a simple,noninvasive, high-sensitivity and recoverable way.
