摘 要:目的 探讨神经肽SP在胎兔皮肤无瘢痕愈合中的作用。方法 建立胎兔背部皮肤全层切割伤模型 ,观察胎兔背部皮肤伤口愈合状况及神经肽SP在无瘢痕愈合中的变化特点。并应用外源性的SP ,观察胎兔伤口愈合情况及神经纤维增生情况。结果 胎兔皮肤愈合快 ,在伤后 3~ 5d所有创伤完全无瘢痕愈合。胎兔皮肤伤后神经肽SP阳性表达降低 ,第 2天最明显 ,为正常胎兔皮肤的 75 % ,然后逐渐恢复正常。成兔皮肤伤后SP表达第 1天明显降低 ,为正常成兔的60 % ,以后明显增加 ,第 7天达峰值 ,为正常成兔的 168% ,其后下降 ,到第 14天与正常成兔基本相似。外源性SP应用后 ,致伤胎兔皮肤瘢痕形成。结论 神经肽SP的低表达是胎兔皮肤无瘢痕愈合机制之一 ;外源性SP可导致胎兔皮肤瘢痕愈合 ,其机制可能与其引起急性神经源性炎症反应和神经纤维的增生有关。Objective To explore the roles of neuropeptide SP in scarless fetal wound healing. Methods The animal model of 22-day-gestational rabbit fetuses (term=31 d) inflicted with a full thickness skin wound on the back was established. The progress of wound healing was observed on at micro- and macro-levels at 1, 2, 3, 5, 7, and 14 d after injury, respectively. The expression level of substance P (SP) during wound healing was detected by immunohistochemical staining. Then the effects of implants containing SP (0.01 mol/L) placed subcutaneously on the fetal wound healing and the proliferation of nerve fibers were observed. Results The cutaneous wounds in fetal rabbits healed more rapidly without the formation of scars than those in adult rabbits. The expression of SP in the wound decreased in fetal rabbit at the early stage after injury, reached the lowest levels at 2 d after injury (75%, vs fetal control), and then increased gradually and reached the normal levels. The expression of SP in the wound decreased in adult rabbits at the early stage after injury, reached the lowest levels at 1 d after injury (60%, vs adult control), and then increased quickly and reached a higher level than that in the normal group. SP expression reached a peak at 7 d after injury (168%, vs adult control), and then returned to the normal levels. Exogenous SP induced acute inflammatory reaction, followed by fibroblast proliferation and collagen synthesis, resulting in a large amount of scars in fetal wound healing. Conclusion The low level expression of neuropeptide may contribute to the fetal scarless wound healing. Exogenous SP may result in fetal wound healing, which may be associated with the acute inflammatory reaction and nerve fiber proliferation.
