摘 要:目的 观察血红素氧合酶1(heme oxygenase,HO-1)与冠心病患者冠状动脉病变及其形态的关系。方法 将经冠状动脉造影最少有一支主要血管腔狭窄≥50%的冠心病患者136例纳入该研究。并选择冠状动脉造影正常的40例患者作为对照组。按照病变血管数量分为单支病变组(61例)和双支病变组(45例)和三支病变组(30例);再按照病变处斑块的形态分为Ⅰ型病变组(53例)、Ⅱ型病变组(70例)、Ⅲ型病变组(13例)。利用免疫组化方法对冠心病患者外周血单个核细胞中HO-1的表达进行定位分析,并通过计算机图像分析系统分析HO-1表达的程度及强度。通过Westernblot对HO-1表达水平进行定量分析。比较冠心病各亚组患者HO-1的表达情况。结果 HO-1表达于胞浆。冠状动脉病变组HO-1表达明显高于冠状动脉正常组(P<0.01);三支病变组HO-1表达明显高于双支病变组及单支病变组(P<0.01),双支病变组HO-1表达也高于单支病变组(P<0.05);Ⅱ型及Ⅲ病变组的HO-1表达水平明显高于Ⅰ型病变组(P<0.01)。结论 冠心病患者HO-1表达水平升高,其表达水平与冠心病严重程度有关。Objective To valuate the association between heme oxygenase-1 (HO-1) and angiographic morphology as well as severity of coronary lesions in patients with coronary heart disease (CHD). Methods One hundred and thirty six patients with CHD who underwent coronary angiography were selected in this study, and 40 patients with normal coronary artery were selected as control. Firstly, according to quantity of coronary lesions, the patients were divided into single vessel group (n = 61) , double-vessel group (n =45) and multi-vessel group (n =30); secondly, according to angiographic morphology of coronary lesions, they were split into type Ⅰ (smooth borders) group (n = 53) , type Ⅱ (irregular borders) group (n =70) and type Ⅲ (long and irregular lesions) group (n = 13). The expression levels of HO-1 protein in monocyte and lymphocyte collected from patients with CHD were tested by immunohistochemistry and western blot. Computer picture analyzing system was also used to measure the expression levels of HO-1 protein. Results HO-1 protein was located in cell plasma; the expression levels of HO-1 protein in patients with CHD were significantly higher than those in the controls (P <0.01) ; in the patients with CHD, the highest expression of HO-1 was in multi-vessel group, and then, in double-vessel group and single vessel group. Also, the expression levels of HO-1 protein in type Ⅱ and type Ⅲ group were significantly higher than that in type Ⅰ group (P <0.01). Conclusions There was a higher expression of HO-1 in patients with CHD, the levels of HO-1 protein may not only reflect the quantity of coronary lesions, but also associate with angiographic morphology of coronary lesions, which suggested that HO-1 may be associated with severity of coronary heart disease.
