摘  要:目的 探讨高敏C反应蛋白(hs-CRP)与代谢综合征组分及其靶器官损害的关系。方法 589例高血压、糖尿病或代谢综合征患者用免疫散射速率比浊法检测血清hs-CRP,分析hs-CRP与血脂、血糖、血压、腹围的关系;同时测定了299例患者颈动脉内-中膜厚度(IMT)、217例左室质量指数(LVMI)和443例尿白蛋白排泄率(UAER)作为判断是否存在代谢综合征靶器官损害的指标,分析hs-CRP与IMT、LVMI和UAER的关系。结果 血清hs-CRP水平随着代谢综合征组成成分数量的增加而升高,当其组成成分≥3个时,hs-CRP水平明显升高。相关分析表明hs-CRP浓度与甘油三酯、腹围、收缩压、舒张压、空腹血糖呈正相关,与高密度脂蛋白胆固醇呈负相关;血清hs-CRP水平与IMT、LVMI和UAER呈正相关关系,当hs-CRP水平升高时,可增加代谢综合征靶器官损害的程度和范围。结论 血清hs-CRP水平不仅与代谢综合征组成成分相关,而且与其靶器官损害程度密切联系。Objective The purpose of this study was to determine the association of the serum levels of high-sensitivity protein (hs-CRP) and components and target organ damage in patients with the metabolic syndrome (MS). Methods Total 589 participants were included in this study. MS criteria was defined by modified NCEP-ATPⅢ. Serum hs-CRP, fasting lipids and blood glucose (FPG), blood pressure, and waist circumference, left ventricular mass index (LVMI) , intima-media thickness(IMT) and urinary albumin excretion rate (UAER) were measured in all the participants. Results Increasing numbers of the MS components resulted in higher serum levels of hs-CRP (x2 = 178.4, P < 0.001). Especially, baseline level of hs-CRP for those with 3 or more MS components was much higher than that for those with 2 or less MS components. Similar finding was observed a parallel change between MS components and its numbers of target organ damage. Moreover, hs-CRP level was highly significant positive correlated with TG, waist circumference, SBP, DBF, FPG, and a significant negative correlation was observed between hs-CRP and HDL-C. Apart from this, there was a statistically significantly positive correlation between hs-CRP and IMT, LVMI and UAER (P < 0. 01). Conclusion The results indicated that serum hs-CRP level was closely related to the components and its target organ damage in patients with MS.
