氟伐他汀对糖尿病肾病患者血Ⅳ型胶原的影响及治疗作用观察

摘　　要：目的观察氟伐他汀对糖尿病肾病(diabetic nephropathy, DN)患者血清Ⅳ型胶原(Ⅳcollagen, CIV)的影响及对DN患者的保护作用。方法60例血脂正常的2型糖尿病患者根据尿微量白蛋白排泄率(urine albumium excretion rate, UAER)分为正常白蛋白尿组(A组)、微量白蛋白组(B组)、临床蛋白尿组(C组),分别检测血糖、甘油三酯(TG)、胆固醇(TC)、低密度酯蛋白(LDL),血肌酐(Cr),UAER和CIV。A、B、C组随机分为氟伐他汀治疗组和常规治疗组。结果糖尿病各组UAER、CIV均显著增高。相关分析表明CIV与Cr,UAER呈正相关。氟伐他汀治疗后UAER、CIV均显著降低。结论糖尿病肾病患者存在胶原代谢异常,氟伐他汀能抑制CIV合成,改善胶原代谢,保护肾脏功能。Objective: To observe the influence of fluvastatin on serum type Ⅳ collagen (CIV) in diabetic nephropathy patients and its theraputic role on diabetic nepropathy. Methods: Sixty cases of type 2 diabetic mellitus patients with normal lipidemia were divided into three groups according to urinie albumium excretion rate (UAER); normoalbuminuria group (A group ), microalbu minuria group (B group ), macroalbu minuria group (C group ). Blood glucose, TG, TC, LDL, Cr, UAER and CIV were detected in all cases. A, B, C groups were randomly divided into group treated with fluvastatin and regular treatment group respectively. Results: In A, B, C group UAER, CIV and significantly increased, CIV was in postive correlation with Cr, UAER. After treatment with fluvastatin in A, B, C groups UAER, CIV and significantly decreased. Conclusion: Abnormal Collagen metabolism exists in diabetic nephropathy patients. Fluvastatin can ameliorate collagen metabolism and protect diabetic mellitus kidney.
