摘  要:目的 探讨甲状腺功能低下的X线改变 ,提高对该病X线改变的认识和诊断。方法 对 3 4例甲状腺功能低下骨龄落后状况、X线改变与甲状腺功能 (TT3 ,TT4)以及智力障碍程度对比分析。对骨龄与骨皮质厚度的关系和正常对照组对比分析 ,了解有无明显骨皮质厚度落后于骨龄的改变。结果 该病骨龄严重落后 ,骨龄与年龄的直线回归方程为y =0 .75 89x -3 .692 8(t =4.417,P <0 .0 5 ) ,骨龄落后与TT3 、TT4低下的程度无明显关系 (P >0 .0 5 ) ,骨骼改变主要是软骨不能正常骨化等引起的一系列X线改变。骨骼异常X线表现的出现与TT3 、TT4低下的程度有关 (P <0 .0 5~ 0 .0 1) ,TT4越低 ,骨骼X线改变出现率越高 (P <0 .0 5 )。智力障碍明显低下者骨龄落后显著 (P <0 .0 5 ) ,用该症的骨龄与正常年龄 (骨龄 )对比皮质厚度无明显改变 (P >0 .0 5 )。结论 本症的骨骼X线改变结合骨龄严重落后可对本症做出诊断。骨骼X线改变 ,及骨龄与年龄的骨皮质改变关系可为临床提供疗效评价依据。Objective To discuss the X-ray findings of hypothyroidism cases so as to promote the recognition and diagnosis of it.Methods Bone age, X-ray features, thyropenia (TT 3,TT 4) and dysgnosia in 34 cases with hypothyroidism were comparatively analyzed. The relationship between the bone age and bone cortical thickness was compared with that in the normal control groups in order to know whether there was bone cortical thickness obviously behind the change of the bone age.Results The bone age was delayed severely in diseased group . The linear regression equation of the bone age ( y ) and age ( x ) was y=0.7589x-3.6928 ( t=4.417, Ρ<0.05) . The delaying of the bone age had no obvious relation with the lower degree of TT 3 , TT 4’s value( Ρ>0.05 ) . The major change of the skeleton was a series of changes caused by cartilage could not be normally ossified.X-ray findings of abnormal skeleton were in relation to the lower degree of TT 3, TT 4 (Ρ<0.05~0.01) .The lower the value of TT 4 was the higher the appearance rate of the skeleton X-ray change was(Ρ<0.05). The delaying of bone age was marked in those cases with obvious dysgnosia (Ρ<0.05) , and the bone cortical thickness had no obvious change by the comparison of the bone age in cases with disease with the normal age (bone age) (Ρ>0.05).Conclusion The definite diagnosis of hypothyroidism can be made based on combining the skeleton X-ray changes with the severe delay of bone age. The X-ray findings of skeleton, the relation of bone age and bone cortical thickness of age can provide infromations in evaluating basis for the clinical treatment effect.
