免疫金银染色检测IgM类抗核抗体及临床意义  

DETECTION OF ANA-IgM BY IMMUNO- GOLD-SILVER STAINING AND ITS CLINICAL SIGNIFICANCE

摘　　要：以胶体金标记羊抗人IgM,对系统性红斑狼疮(SLE)、类风湿关节炎(RA)等病患者160份血清和40份正常人血清作免疫金银染色,以检测IgM类抗核抗体(ANA-IgM),并与IgG类ANA作对照比较。结果表明,在RA,ANA以IgM类为主,检出率88.9％,高于IgG类的63％,滴度则两类相当,对RA的特异性以IgM类高;在SLE,ANA则以IgG类为主,尤其活动性者IgG类检出率(100.0％)与滴度显著高于IgM类(检出率82.1％),对SLE的特异性两类相当。ANA-IgM在有重要组织脏器损害的SLE的检出率显著高于无损害者,还可出现于严重肾损者尿中。故临床检测ANA-IgM很有意义。Gold-conjugated sheep anti-human IgM was -prepared and used to deted the ANA of IgM class ( ANA-IgM ) the by immunogold- silver staining ( IGSS ) in the sera from 160 patients with donnective tissue diseases ( SLE, rheumatoid arthritis ( RA ) and so on ) and otherdiseases, and in that from 40 normal persons. The results of ANA IgM were compared with that of ANA of IgG class (ANA-IgG). It was found that in RA, ANA were mainly of IgM class. The positive rate of ANA-IgM ( 88.9% ) was obviously higher than that of ANA IgG (63% ) ,the liter of ANA-IgM was about equal to that of ANA-IgG and the specificity of ANA-IgM to RA wag higher than that of ANA-IgG. In SLE, the ANA was mainly classed as IgG, especially in its active phase, the positive rate and titer of ANA- IgG were obviously higher than those of ANA-IgM, bul the specificity of the 2 classes of ANAwere similar. ANA-IgM could be easily found in the serum of SLE patienls whose important tissues or organs were damaged and in the urine when the patient's kidneys were seriously damaged. The authors consider it clinically significant to,detecl ANA-IgM.
