再生障碍性贫血41例临床分析

摘　　要：目的　总结 4 1例再生障碍性贫血 (再障 )患者临床发病特点、诊断及治疗效果。方法　对 10年间确诊的 4 1例再障患者的临床资料进行回顾性分析。结果　 4 1例再障患者中慢性再障的治疗有效率为 80 % ,重型再障为 4 2 9%。慢性再障中单用雄激素治疗有效率 76 9% ,CSA和雄激素联合应用的有效率为 83 3% ;重型再障中单用雄激素治疗有效率为 2 5 % ,CSA和雄激素联合应用的有效率为 5 0 %。院内死亡 5例中 ,3例死于脑出血。结论　雄激素是治疗慢性再障的主要药物 ,雄激素和CSA联合应用可提高疗效 ,单用雄激素治疗重型再障效果不佳 ,主张用CSA、ATG治疗 ,或CSA、ATG、大剂量甲基强的松龙、造血细胞因子联合应用。Objective To analyze the clinical characteristics and diagnosis and treatment outcome in 41 cases with aplastic anemia (AA). Methods We registered patients newly diagnosed AA between 1993 to 2003 using questionnaires. Retospective analysis was used. Results Of the 41 cases,treatment effective rate was 80% of chronic AA,42.9% of severe AA.In chronic AA,the rate was 83.3% in treatment with cyclosporine A combined androgen and 76.9% with androgen alone,In severe AA,the arte was 50% with cyclosporine A combined androgen versus 25% with androgen alone.The main cause of death was brain bleeding (60%). Conclusion Androgen was a main drug for chronic AA.Androgen combined with cyclosporine A could increase the effective rate.But for severe AA,androgen was not an effective drug.Cyclosporine A or ATG was recommended for severe AA.Cyclosporine A combined with ATG and high does methylprednisolone and hematopoietic cytokine would improve the trearment outcome.
