彩色多普勒超声对睾丸扭转与急性附睾-睾丸炎的鉴别诊断

摘　　要：目的 :探讨彩色多普勒超声对睾丸扭转与急性附睾 -睾丸炎鉴别诊断中的价值。方法 :首先用二维超声显示 14例睾丸扭转与急性附睾 -睾丸炎的睾丸、附睾的位置、大小、形态、回声 ,继之用彩色多普勒(CDFI)观察睾丸、附睾内的血供情况 ,其诊断均经手术或临床证实。结果 :4例睾丸扭转患侧睾丸、附睾内血供减少或消失 ,10例急性附睾 -睾丸炎血供增多。 1例超声检查过程中自行复位后的血供增多 ,易误诊为急性附睾 -睾丸炎。结论 :CDFI作为一种经济、实用、准确、无创性的检查手段 ,是目前鉴别睾丸扭转与急性附睾炎的首选方法。Objective:To investigate the value of c olor Doppler ultrasound(CDU)in the differential diagnosis of testicular torsion from epididymo-testitis Methods:Fourteen patients with suspected torsion of testi s and acute epididymo-testitis were examined by 2D Ultrasound first to display the position,size,form,echo of the testis and epididymus,and then the blood perf usion was observed by CDU The results were compared with those of operation and clinical Results:Four patients without or with reduced testicular a nd epididymal blood flow were diagnosed as testicular torsion Ten cases with ac ute epididymo-testitis revealed blood flow increasing One case with blood flow increase after spontaneous torsion reduction was misdiagnosed as acute epididym o-testitis during reaxamination Conclusions:As a kind of economic,practical,accurate and s uccessful means of examination,CDFI is the first-selected method to distiguish torsion of testis from acute epididymo-testitis
