年龄及月经周期时相对不成熟卵母细胞质量及体外成熟的影响

摘　　要：目的　探讨年龄和月经周期时相对不成熟卵母细胞质量及其体外成熟 (invitromaturation ,IVM )能力的影响。方法　对 30例子宫肌瘤、子宫内膜异位症、腺肌症患者的 134个生殖泡 (germinalvesicle,GV)期不成熟卵母细胞进行体外培养 ,观察成熟率、受精率 ;按卵泡期、黄体期和不同年龄组进行分析研究。结果　随年龄增长采集的卵母细胞数目明显下降 ;卵泡期和黄体期采卵数目无差异 ;但卵泡期采集的卵母细胞受精率明显高于黄体期采集的卵母细胞 (P <0 0 1)。结论　大于 39岁妇女的卵巢不推荐作为不成熟卵母细胞的来源 ;若采集不成熟卵母细胞在卵泡期进行优于黄体期。Objective To investigate the effects of donor's age and retrieval menstrual cycle phase on the quality of immature oocytes and the results of in vitro maturation. Methods 134 germinal vesicle stage oocytes aspirated from antral follicles of 30 women undergoing gynecologic operation for nonovarian disease, aged from 23-43 years old, were cultured in vitro and the GVBD rate, MⅡ stage rate and fertilization rate were observed. The results were analyzed according to the age group, follicular phase and luteal phase. Results The number of retrieved immature oocytes was declined significantly with an increase of age. The mean number of immature oocytes in 23-35 age group (group Ⅰ), 36-39 (group Ⅱ), 40-43 (group Ⅲ) was 5 857±0 899, 4 857±0 770 and 2 778±0 441 respectively with the difference being significant among the three groups ( P <0 05 for Ⅰvs Ⅱ, P <0 01 for Ⅱ vs Ⅲ, P <0 01 for Ⅰ vs Ⅲ). There was no significant difference in GVBD rate, MⅡ stage rate and fertilization rate among the three groups ( P >0 05). The mean number of immature oocytes retrieved at follicular and luteal phase showed no significant difference ( P >0 05), so GVBD rate and MⅡstage rate ( P >0 05). However, the fertilization rate at follicular phase was significantly higher than that at lutael phase. Conclusion The retrieval of immature oocytes should be carried out from those women under 39 years of age. Immature oocytes retrieved at follicular phase showed higher fertilization rate than at luteal phase.
