摘 要:为探讨严重急性呼吸综合征 (SARS)病毒 (SARS- Co V)抗体在 SARS病原学诊断中的特异性 ,应用酶联免疫吸附试验 (EL ISA)和荧光定量 RT- PCR技术检测了 80例非 SARS患者 SARS- Co V抗体的阳性率。结果在 2 3例健康人中 ,SARS- Co V- Ig G抗体的阳性率为 8.7% (2 /2 3) ,2 0例肿瘤患者中 ,阳性率为 2 0 % (4 /2 0 ) ;18例自身免疫性疾病患者中 ,SARS- Co V- Ig M抗体和 SARS- Co V- Ig G抗体阳性率分别为 11.1% (2 /18)和 2 2 .2 % (4 /18) ;19例系统性红斑狼疮 (SL E)患者中 ,SARS- Co V- Ig M抗体和 SARS- Co V- Ig G抗体阳性率分别为 2 1.1% (4 /19)和 4 7.4 % (9/19) ,SARS- Co V- Ig G抗体和 SARS- Co V- Ig M抗体同时阳性率为 15 .8% (3/19)。证实 SARS-Co V- Ig M抗体诊断 SARS的特异性为 92 .5 % ;SARS- Co V- Ig G抗体诊断 SARS的特异性为 76 .2 5 % ;两种抗体同时阳性诊断 SARS的特异性为 96 .2 5 %。经 RT- PCR证实上述抗体单项阳性均为假阳性。认为两种抗体同时测定可提高诊断的特异性 ,出现假阳性的原因可能与包被的抗原中存在细胞膜、胞浆。To discuss the specificity of serology diagnostic testing for coronavirus antibody in the patients with severe acute respiratory syndrome(SARS) 80 individuals with non SARS were detected for SARS coronavirus (SARS CoV) antibody and RNA with enzyme linked immunosorbent assays(ELISA) and reverse transcriptional polymerase chain reaction(RT PCR) The results showed that in the 23 cases healthy controls,2/23 cases(8 7%) were positive of SARS CoV IgG antibody In 20 cases with cancer,4/20(20%) were positive of SARS CoV IgG antibody In 18 cases with autoimmune disease,positive rates of SARS CoV IgG and IgM antibody were 22 2%(4/18) and 11 1%(2/18),respectively In 19 cases with SLE,positive rates of SARS CoV IgG and IgM antibody were 47 4%(9/19) and 21 1%(4/19),respectively,and 3 cases(15 8%) were double positive of SARS CoV IgG and IgM antibody The specificity of SARS CoV IgG and IgM antibody for SARS were 76 25% and 92 5%,respectively,but the specificity of SARS CoV IgG and IgM antibody for SARS was 96 25% at the same time The positive of SARS CoV IgG and IgM antibody were false positive by RT PCR This results suggests that the specificity of ELISA tests for SARS is excellent if using SARS CoV IgG and IgM antibody tests A possible cause of false positive with SARS CoV IgG and IgM antibody is coated antigens with SARS CoV and Vero E6 cells in ELISA methods
