妊娠高血压综合征患者胎盘基质金属蛋白酶-9及其组织抑制物-1的表达及胎盘病理改变

摘　　要：　目的　研究基质金属蛋白酶 (matrix metalloproteinases, MMP) 9 及其组织抑制物 (tissue inhibitor ofmetalloproteinases, TIMP) 1在妊娠高血压综合征 (妊高征) 患者胎盘中的表达, 探讨其与妊高征发病的关系。方法应用原位杂交法对 37例妊高征 (轻度17 例, 中、重度 20 例) 患者胎盘中 MMP- 9 及 TIMP- 1 mRNA进行检测。同时做苏木精 伊红染色观察妊高征时胎盘的病理改变。结果　MMP- 9、TIMP- 1 主要表达于滋养细胞、绒毛间质血管壁及蜕膜组织中; 中、重度妊高征组MMP- 9 mRNA, TIMP- 1 mRNA的表达为 (0 038±0. 074, 0 .080±0 018) 明显低于正常妊娠组的 (0 .065±0 012, 0 .097±0 .029) 及轻度妊高征组的 (0. 065±0 .015, 0. 093±0 .025), 分别比较差异均有显著性意义; 轻度妊高征组与正常妊娠组的 MMP 9 mRNA 表达差异无显著性意义 (P> 0 .05), 而TIMP 1 mRNA表达差异有显著性意义; 中、重度妊高征 MMP 9/TIMP 1 比值 (0. 455±0. 046) 明显低于正常妊娠组 (0 741±0 080) 和轻度妊高征组 (0 .679±0. 010), 差异均有极显著性意义 (均 P<0 .01); 中、重度妊高征组胎盘绒毛数、间质血管数 [ (7. 25±1. 74)、(15. 83±2. 04) 个/高倍视野] 均明显低于正常妊娠组 (均P<0. 01)。Objective To investigate the relationship between the messager RNA expression of matrix metalloproteinase-9(MMP-9) and tissue inhibitor of metalloproteinase-1(TIMP-1) in placenta of the patients with pregnancy-induced hypertension (PIH).Methods In placenta from 20 cases of normal pregnancy and 37 cases of mild, moderate and severe group of pregnancies complicated with PIH, the expression of MMP-9/TIMP-1 mRNA were examined by in situ hybridization.The placental pathology change was observed by hematoxylin and eosin stain.Results MMP-9 and TIMP-1 mRNA were expressed in trophoblast or vassal wall of villous stroma or decidual tissue.In moderate and severe PIH, the expression of MMP-9(0.038±0.074)and TIMP-1 mRNA(0.080±0.018) and the ratio of MMP-9/TIMP-1(0.455±0.046) were significantly lower than that in normal pregnancy [(0.065±0.012),(0.097±0.029),(0.741±0.080)]and mild PIH[(0.065±0.015),(0.093±0.025),(0.679±0.010)](P<0.01)respectively.But the differences of expression of MMP-9 mRNA and ratio of MMP-9/TIMP-1 in normal pregnancy and mild PIH were not significant(P>0.05).In moderate and severe PIH,the number of villi(7.25±1.74) and vessel(15.83±2.04) was reduced significantly(P<0.01).Conclusion Decreased expression of MMP-9 and imbalance in MMP-9/TIMP-1 are related to invasive defect, abnormal vascular remodeling and PIH.These abnormal changes may be responsible for the placental pathological change.
