2型糖尿病患者的认知功能与餐后血糖及病程的关系

摘　　要：目的:调查2型糖尿病患者认知功能表现,寻找其规律性。方法:2002-02/2003-02在深圳市第二人民医院院门诊就诊和住院治疗的2型糖尿病患者137例。符合标准2型糖尿病患者95例,95例患者均接受了中国韦氏成人智测量表(WAIS-RC)评估,并与正常人常模数据进行对照。结果:95例2型糖尿病患者知识(17.3±5.1),算术(6.8±2.6),相似性(12.5±4.8),词汇(51.6±18.3),图形排列(16.4±5.9)测验得分明显低于正常人常模(t=2.241～3.176,P<0.05～0.01)。餐后血糖高(≥11.0mmol/L)患者的知识、算术、相似性、数字广度、词汇、图形排列等分量表评分明显低于餐后血糖正常或偏高(≥7.8mmol/L,<11.1mmol/L)患者(t=2.131～3.182,P均<0.05)。病程长的患者知识、领悟、算术、相似性、词汇、图形排列等分测验值明显低于病程短的患者(t=2.143～3.251,P均<0.05)。结论:2型糖尿病患者常有认知功能障碍表现,该认知功能障碍特征与患者的病程和餐后血糖浓度相关。AIM:To investigate the cognitive function of type 2 diabetes mellitus(DM) and seek the regularity. METHODS:Totally 137 patients with type 2 DM were selected from the outpatients and inpatients in the Second People's Hospital of Shenzhen from February 2002 to February 2003.Ninety five met the criteria,and received the evaluation of Wechsler adult intelligence scale Chinese revision(WAIS RC) for comparison with the norm. RESULTS:In the WAIS RC assessment, the scores of type 2 DM patients on information(17.3±5.1),arithmetics(6.8±2.6),similarities(12.5±4.8),vocabulary(51.6±18.3),and picture arrangement(16.4±5.9) were significantly lower than those of the norm(t=2.241 to 3.176,P< 0.05 to 0.01).The scores on information, arithmetics,similarities,digit span,vocabulary,picture arrangement,etc.were significantly lower in the patients with high postprandial blood glucose(PBG)(≥11.0 mmol/l) than those with normal or a little high PBG(≥7.8 mmol/L,< 11.1 mmol/L)(t=2.131 to 3.182,P< 0.05).Patients with long disease course had lower scores on information,comprehension,arithmetics,similarities,vocabulary,picture arrangement,etc.than those with short disease course(t=2.143 to 3.251,P< 0.05). CONCLUSION:Type 2 diabetic patients often have cognitive dysfunction,and their cognitive dysfunction is related with their disease course and PBG.
