揭示神经病理性疼痛机制:坐骨神经分支选择性损伤模型的研究进展

摘　　要：目的:为研究神经病理性痛的机制,能够模拟神经病理性痛的临床特点的实验动物模型显得尤为重要。综述了近几年发展起来的坐骨神经分支选择性损伤(sparednerveinjury,SNI)所致神经病理性痛的研究进展。资料来源:应用计算机检索Pubmed1990-10/2004-10期间的相关文章,检索词neuropathologicpain,nerveinjury”,再利用检索词spared““nerveinjury”进行检索,文章语言限定为English。同时手工或计算机检索万方数据库1990-10/2004-10期间的相关文章,限定文章语言种类为中文,检索词“神经病理性痛、周围神经损伤”。资料选择:对资料进行初审,选取与周围神经损伤所致病理性痛密切相关的研究。纳入标准:研究对象为动物,包括坐骨神经的松散结扎/慢性挤压、部分坐骨神经紧密结扎、腰L5～6脊神经根切断术和SNI等研究内容。重点选取与SNI研究相关的文献。排除标准:综述性论文。资料提炼:共收集到85篇相关文献,其中与SNI直接相关的文献8篇,间接相关的文献25篇,排除重复的12篇,共21篇用于资料的综述。资料综合:对选取的文献进行阅读、归纳及综合。综合的结果显示,SNI模型的手术易于操作可重复性好,术后动物的行为表现能够模拟部分神经病理性痛的特点且持续时间长,不同镇痛药物对SNI诱导的神经病理性痛行为反应作用不同?OBJECTIVE:In order to investigate the mechanisms of neuropathologic pain,the m odels of experimental animal,which can imitate the clinical characteristics of n europathologic pain,are very important.This article summarizes the research prog ress of neuropathologic pain induced by spared sciatic nerve injury(SNI) in rece nt years. DATA SOURCES:Using the key terms neuropathic pain,nerve injury,we searched the Pubmed database of the relevant references in English language from October 199 0 to October 2004.Another key word “spared nerve injury ' was also used.Meanwhi le,using the above key terms in Chinese,we searched Wanfang database in Chinese language from October 1990 to October 2004. STUDY SELECTION:All articles were selected firstly,those were relevant to neur opathologic pain induced by peripheral nerve injury were selected.Inclusive crit eria:As the subjects,many aspects of animals were researched,including loose lig ation/chronic extrusion of the sciatic nerve,tight ligation of part sciatic nerv e,rhizotomy of lumbar 5 and 6 spinal root as well as SNI,with the emphases on SN I.Excusive criteria:The summarizations were excluded. DATA EXTRACTION:Totally 85 articles were collected.Among them,eight articles w ere closely related with SNI,25 articles were indirectly related with SNI,12 dup licated articles were excluded, and totally 21 articles were used for the summar ization. DATA SYNTHESIS:Careful reading,summary and integration were carried out to tho se selected articles.The results showed that the operation of the SNI model was easy to perform and with good reduplication.The behavior of animals could imitat e the clinical characteristics of the neuropathologic pain and last a long perio d.Different analgesic drugs showed different effects on the neuropathologic pain induced by SNI. CONCLUSION:SNI model has different characteristics with other models of neurop athologic pain.It is a new model to study the mechanisms of neuropathologic pain .
