两种短程三联方案治疗飞行员幽门螺杆菌阳性消化性溃疡疗效分析

摘　　要：目的　研究呋喃唑酮 (F)或甲硝唑 (M)联用阿莫西林 (A)及枸橼酸铋钾 (C) 1周疗法治疗飞行员幽门螺杆菌 (Hp)阳性消化性溃疡的疗效。方法　 60例飞行员经胃镜证实患有直径≥ 3 mm的活动期消化性溃疡 ,随机分为治疗组和对照组 ,每组各 3 0例。治疗组 (CAF )给予呋喃唑酮 0 .1g,3 /日 ;联用枸橼酸铋钾 0 .2 2 g,2 /日 ;阿莫西林 1g,3 /日。对照组 (MAF)给予甲硝唑 0 .4g,3 /日 ,联用枸橼酸铋钾 0 .2 2 g,2 /日 ;阿莫西林 1g,3 /日。疗程均为7日。结果　溃疡愈合率 ,HP根除率治疗组分别为 93 %、93 % ,而对照组分别为 76%、70 % ,两组比较差异有显著性。治疗 1日、3日、7日缓解腹痛显效率治疗组分别为 63 %、90 %、97% ,对照组分别为 5 3 %、69%、76% ,两组比较差异有显著性。两组均无严重不良反应 ,患者依从性较好。结论　 CAF疗法较 MAF疗法疗效好 ,不良反应少 ,依从性好 。Objectives To study the efficacy of one week triple therapy with furazolidone or metironizole combined with am oxycillin and bismuth potassium citrate for the treatment of helicobacter pylori in fection in active degest ulcer.Methods 60 pilots cases of degest ulcer were randomly delivered to CAF group,in which patients were given bismuth potassium citrate 0.22 g bid+furazolidone 0.1 g tid +amoxycillin 1.0 g tid for one week,and MAF group as a control,in which patients were given bismuth potassium citrate 0.22g bid +metironizole 0.4g tid +amoxycillin 1.0 g tid for one week.Results ulcer cure rate and H.pylori eradication rate after CAF treatment were 93% and 93%,while 76% and 70% after MAF treatment.The relief rate of abdominal pain at 1 day,3 day and 7 day after CAF treatment were 63%,90% and 97%,while 53%,69% and 76% after MAF treatment.there were significant differences in ulcer cure rate,H.pylori eradication rate and the relief rate of abdominal pain between the two groups.there were no severe side effects in both group.Conclusions one week triple therapy of CAF appears to be more effective in active degest ulcer with helic obacter pylori in fection than that of MAF.CAF regimens seems to have better tolerance and compliance.
