国产双异丙酚复合氯胺酮用于小儿麻醉可行性研究

摘　　要：目的　观察国产双异丙酚复合氯胺酮用于小儿麻醉的可行性。方法　 30例小儿手术均采用微量泵静注复合液 (含双异丙酚、氯胺酮各 5mg·mL- 1 )完成。观察术前、切皮后 1min、15min、30min、6 0min及缝皮后 1min的收缩压 (SP)、舒张压(DP)、脉率 (P)、呼吸频率 (RR)及未吸氧条件下末稍血氧饱和度 (SpO2 ) ,同时观察术后病人的睁眼时间 ,定向力恢复时间及其伴有的副作用。结果　双异丙酚、氯胺酮复合液用于小儿麻醉的诱异剂量为 1.13 + 0 .2 6mg- 1 ·kg- 1 ,维持剂量为 4.46 +2 .17mg·kg- 1 ·h- 1 。SP、DP、P、RR、SpO2 于术后各时点与术前相比没有显著变化 ,术后睁眼时间为 19.0 7+ 8.99min ,定向力恢复时间为 33.70 + 13 .77min ,恶心呕吐发生率为 10 % ,复视发生率为 16 .7% ,除此之外无其他不适。结论　国产双异丙酚复合氯胺酮用于小儿手术是一种安全 ,有效 ,苏醒快 。Objective To observe the feasibility of Chinese made propofol and ketamine mixture used in children anaethesia.Methods Of 30 surgical operations in children, we injected mixed liquid(propofol 5mg·mL -1 and ketamine 5mg·mL -1 ) into the veins.At the time of preoperation,after incision for 1min,15min,30min,60min and 1min after sewing the wound,we recorded the systolic pressure(SP),diastolic pressure(DP),pulse(P),respiratory rate(RR),and finger oxyhemoglobin saturation(SpO 2) without breathing oxygen.The time when the patients opened their eyes,when the patient recovered the ability of direction and the following side effects were cbserved.Results Induction dose of both propofol and ketamine in children operations in 1.13+0.26mg·kg -1 ,maintaining dose is 4.46+2.17mg·kg -1 ·h -1 .Comparing with preoperation SP,DP,P,RR,SpO 2 during opertion and after operation have not evidently changed.The mean time for opening eyes in 19.07+8.99min. The mean time for recovering directional ability is 33.70+13.77min.Rate of nausea and vomit is 10% in the patients.Rate of double-sight is 16.7%.There is no other unwell except for those outlined above.Conclusion In children operations Chinese-made propofol mix ketamine is a safe,effective,fast revival and little side effect anaethetic.
