体外循环前后血小板功能变化的临床研究

摘　　要：分时段测定 2 0例心脏直视手术患者体外循环 (CPB)前后血小板计数、血浆α-颗粒膜蛋白 (GMP-140 )、血栓烷 B2 (TXB2 )、6 -酮 -前列腺素 F1 α(6 - K- PGF1 α)变化值。结果血小板计数、GMP- 140、TXB2 测值随着CPB进行均逐渐增加 ,停机后 1h与肝素化 10 min相比 ,GMP- 140极显著增加 (P<0 .0 1) ,TXB2 显著增加(P<0 .0 5 ) ,血小板计数极明显降低 (P<0 .0 0 1) ,而 6 - K- PGF1 α在 CPB前后变化不明显。认为体外循环中血小板功能受到明显损害 ,GMP- 140和 TXB2是从分子水平反映血小板功能损害的敏感指标。To probe variety of function of platelet around CPB and its clinical significance,Platelats count,GMP 140,TXB 2,6 K PGF 1α were detected in 20 cases of open heart surgery on 4 time points. The results showed Platelats count were very obviously reduction.The detected quantity of GMP 140,TXB 2 were all obviously greater after CPB 1h than that on the time after heparinization 10 min;There were no obviously variety in 6 K PGF 1α around CPB. these suggested that the function of platelet were destroied obviously around CPB; GMP 140 and TXB 2 are sensitive targets of reflecting active damage of platlete.
