摘　　要：乙状结肠法人工阴道成形术术式较复杂,为使手术成功应严格掌握:选取肠段的部位以平骶骨岬的高度作为远侧端切断部位为宜;切取肠段的长度一般为16～18cm;为保证移植肠段的血供,选留移植肠段系膜内的血管应远离下植端,避免下植困难或下置后牵拉过紧影响血供。为使人工阴道外口成形成功,采用下植肠段断端缝合闭锁线不拆,而于其稍下方沿结肠带另开口的方法、为减少膀胱及直肠的副损伤,手术步骤采用先开腹切取移植肠段,再行人工阴道造穴的阴腹联合操作,以利手术顺利成功。The operation of artificial vaginoplasty with sigmoid has obvious advantages in many aspects, such as elimination of using vaginal dilators after the operation, softness of the vagina with good elasticity and secretion function, and enough extent of the vagina. The author reported 588 cases of this operation with 98.92% rate of success. The key to the succes was strict preoperative preparation of the intestinal tract and ideal management before and after the operation. In order to assure the blood supply of the intestinal graft, the distal cutting sire should he selected at the sacral promontory level, and reserve the respective mesenteric vessels far from the inferior extremity of the graft. Many other surgeons did fistulization prior to the resection of the intestinal segment, but the author prefers resection of the intsetinal segment before the fistulization of the artificial vagina. The advantage is that the vaginal and the abdominal operation could be cooperated with each other, and injury of the bladder and the rectum will be decreased.
