大隐静脉剥脱术避免隐神经损伤的解剖基础

摘　　要：解剖观察了50例正常成人小腿段的大隐静脉与隐神经的位置关系。小腿上1/3段二者之间有0.5～3.0cm的距离,中、下1/3段二者紧密伴行,神经可位于静脉的前方、后方或深面,也可缠绕或分支夹持静脉。在静脉和神经的外周有血管神经鞘包绕。因此,依据解剖学研究,在大隐静脉剥脱术时,为了避免损伤隐神经,小腿上1/3段以上应向上抽剥,中、下1/3段应向下抽剥。The positional relation between the saphenous nerve(SN)and the long saphenous vein(LSV)was observed in 50 leg specimens of adult cadavers.The LSV is accompanied by the SN inthe whole course in general,but they are separated for 0.5～3.0cm in distance in the proximal1/3 section of the leg.In the distal 2/3 sections,they are closely wrapped together by a connec-tive tissue sheath,and the SM may lie anterior,posterior or deep to the LSV,or it may twine orclip the LSV by its branches.Therefore,it was anatomically recommended that the varicose LSVin the proximal 1/3 section of leg should be stripped upward and that in the distal 2/3 sectionsstripped downward to avoid the avulsion of the main trunk or branches of the SN.
