老年性COPD患者核素心血池显像的心功能测定及评价

摘　　要：对57例老年COPD患者和37例正常人进行核素心血池显像。结果表明,正常人组中老年组与中青年组比较,前者以左心TPFR、PFR、TPE参数明显下降。老年COPD并肺心病两组RVEF、RVSDP均较正常老年对照组明显下降和增宽。尤以RVSDP改变较早。左心LVEF、EPFR、PFR、1/3FF在肺心右心衰竭组才发生改变。提示:(1)正常人随年龄增加,心搏出量及快速充盈率下降;(2)右心RVEF和RVSDP对判断COPD合并早期肺心病是较灵敏的指标。肺心病心功能失代偿时,左心收缩及舒张功能也均受损。The phase analysis was examined in 57 cases of elderly COPD patients and 37 normal subjects using the radionuclide gated blood pool image. It was shown that the parameters of TPFR, PFR, TPE of left ventricule in normal elderly subjects were significantly decreased than those in normal middle young group. The RVEF were more significantly decreased and the RVSDP(S.D of RV phase distribution) were more significantly increased in elderly patients with COPD complicated with cor pulmonale than those in the normal elderly group. The whole LVEF, TPFR, PFR and 1/3FF of left ventricule remained unchanged in cor pulmonale unless right heart failure occurred, We considered: (1) the older the normal person, the lower the stroke volume and rapid peak filling rate; and (2) both the RVEF and RVSDP may be taken as a sensitive index for diagnosis of COPD complicated with early cor pulmonale. When heart function of cor pulmonale decompensated.both the systolic and diastolic function of left ventricule were impaired.
