脊柱腰段外侧区的解剖及其临床意义

摘　　要：目的:为腰椎间盘穿刺摘除髓核的进针部位提供解剖学资料。方法:解剖观察成人尸15具(男11、女4)脊柱腰段外侧区血管神经的配布及无血管神经区的位置与范围。结果:脊柱腰段外侧无血管神经区的范围L_1～L_5为正方形,长(上下距离)最小均值为25.03±3.46mm,最大均值为27.52±5.56mm;宽(前后距离)最小均值为23.75±6.67mm,最大均值为32.81±4.37mm;L_5～S_1区为三角形,长平均为16.42±3.80mm,宽平均为13.63±3.37mm。结论:经L_1～L_5外侧区的椎间盘外侧面进针行相应髓核摘除术是安全的。L_5～S_1外侧无血管神经区范围较小,进针较困难。Objective: To provide anatomical data for safe lumbar intervertcbral disc insertion route to resect necleus pulposus. Methods : The distribution of vessel and nerve and the range of nonangioneuro-area in lateral region of lumbar vertebrae were observed on 15 adult cadavers (male 11, female 4). Results :The nonangioneuro-area in the lateral region of lumbar vertebrae which ranges form L1 - L5,is rectangular. It s minimum average length is 25. 03 ± 3. 46mm, maximum average length 27. 52 ± 5. 56mm; minimum average width 32.81 ± 4.37mm. L5 - S1 is triangular, with an average length of 16.42 ± 3. 80mm;an average width of 13.63 ± 3.37mm. Conclusion : It is safe to insert from the lateral region of lumbar vertebrae(from L1 - L5)gor percutarious resectior of nucleus pulposus, it' s difficult to insert from L5 -S1,as the norangioreuro-area in that region is small.
