胸血自体回输后血液成分变化及对肾肝肺功能影响的研究

摘  要:目的:探讨胸血回输后血液成分变化及对肾肝肺功能的影响,旨在为胸血自体回输提供科学的实验室依据及实用的操作模式。方法:对30例胸血进行回输,回输血量500~2 000ml,平均每例979ml。同时对胸血及其回输后血液成分变化以及对肾肝肺功能的影响,作了系列检测和分析。结果:胸血质量近似体循环血,胸血及体循环血一般细菌培养均为阴性,回输血过程无过敏、溶血反应、凝血机制紊乱及其它血行感染,临床观察及多次检验表明,回输前后患者的β2微球蛋白、血气分析、肝功能等多项反映肾肝肺功能的指数无明显改变,获满意输血效果。结论:胸血自体回输安全可行,对患者肾肝肺功能无明显异常影响。胸血回输为抢救危重病人提供了一种安全、简便、经济的血源。To evaluate the variations of blood c omponents and the effe cts on renal, liver and lung functions, and to establish the scientific laborato ry proof and the practical operating mode Methods: 30 pati ents underwent autotran sfusion of thoracic blood with a mean autotransfusion magnitude of 979ml (range, 500ml to 2000ml) The components of thoracic blood, the variations of blood co m ponents after autotransfusion and the effects on renal, liver and lung functions were examined and analysed Results: The components of t horacic blood were akin to body blood The results of common bacterial culture of thoracic blood and b o dy blood were negative There were not allergic reaction, hemolytic reaction, d y sfunction of blood coagulation and hematogenous infection It disclosed that th e parameters of renal, liver and lung function (including β2MG, blood gas ana lysis, liver function, et al) did not grossly vary after autotransfusion These patients responded well to autotransfusion Conclusion: Autotransfusion of thor acic blood is safe and it has few effects on the functions of renal, liver and lung The thoracic blood is a safe, simple, economical blood resource for salvag ing the serious patients
