彩色多普勒血流图探测三尖瓣环缩术前后返流量的变化

摘　　要：用彩色多普勒血流图(CDFI)检查122例风湿性心脏瓣膜病患者行二尖瓣和/或主动脉瓣人工瓣置换术加三尖瓣环缩术前后三尖瓣返流(TR)的变化。用收缩期TR的瞬时面积(TR—IA)分级,与彩色多普勒频谱测得的每搏返流容量(TR—SV)对照,发现TR—IA<2cm^2,2～4cm^2,4.1～10cm^2,>10cm^2时,TR—SV分别为0.81±0.23ml,3.68±1.19ml,8.67±3.06ml,30.47±14.32ml。本研究说明,CDFI探测TR是一种非侵入性的、敏感性高、特异性强的诊断方法。The tricuspid regurgitation (TR) of 122 patients with rheumatic heart valvular diseases, whose mitral and/or aortic valve replacements and tricuspid annuloplasty were performed, were measured by color Doppler flow imaging(CDFI) before and after operation. By comparing and analysing relationship between the instantaneous areas of TR (TR—IA)during systole and stroke volume of TR (TR—SV)calculated with color Doppler frequency spectra, we found that when the TR—IA was<2 cm^2, 2～4 cm^2, 4.1～10 cm^2, >10 cm^2, the TR—SV was 0.81±0.23 ml, 3.68±1.19 ml, 8.67±3.06 ml, 30.47±14.32 ml, respectively. This study suggested that CDFI examination is a noninvasive high—sensitive and specific method for diagnosis of TR.
