摘 要:肝细胞坏死是病毒性肝炎病变特点上一,但其坏死机理尚未阐明。为此,本文以气球样肝细胞为重点,用电镜、组化及电镜细胞化学等方法观察SAH、CAH、CPH及GalN致大鼠肝细胞损伤。结果显示气球样肝细胞主要表现为线粒体肿胀,内质网扩张,胞质局灶性溶解及质膜受损,伴相应酶活性下降,包括Na^+-K^+-ATP酶、G-6-P酶、SDH等。由于质膜损伤,Na^+-K^+-ATP酶活性下降,可导致细胞内Ca^(2+)浓度增加,后者激活磷脂酶等,严重破坏膜结构,致使细胞死亡。本文用ARS染色证实了气球样细胞Ca^(2+)浓度增加,提示病毒性肝炎细胞坏死可能与Na^+-K^+-ATP酶活性下降,细胞内Ca^(2+)In order to study the early pathological changes of cellular necrosis in viral hepatitis, the injured hopatocytss were examined by means of histochemif al and electronmicroscopieal techniques. The Samples were obtained from patients of SAH, CAH, CPH and from rats treated with D-GalN, The observation was focused on cellular balloon degeneration, the results showed the characteristics of mitochondr-ial swelling, endoplasmic reticulum dilatation, cytoplasmic focal lysis and plasm-alemmal lesion with decrease of related enzymatic activities, including N?-K+-ATPase, G-6-Pase, SDH, etc.The lesion of plasma membrane and decrease of Na+-K+-ATPase led to cellular swslling and rise of Ca2+ concentration in cytoplasm, The latter activated phospho-lipases which destroyed the membrane structures, aad finally brought about disintegration and death of cells, the increasing Cai+ concentration in balloon hcpatoc-ytes was confirmed by ARS staining.These results suggest that the mechanism of hepatocellular necrosis in viral hepatitis may be related to intracellular accumulation of calcium ions
