改良薛氏位摄片对颞下颌关节紊乱综合征诊断价值的探讨

摘  要:通过52例(104侧关节)颞下颌关节紊乱综合征患者采用薛氏位和改良薛氏位颞下颌关节投照方法的对比研究,提出以关节前间隙作为关节盘移位的预测指标,并以造影结果作为金标准,最后经统计学分析,结果发现,同一病例两种不同方法投照有高度显著性差异,改良薛氏位前间隙增宽是一理想的预示盘前移位的指标。It is necessary to use X-ray in diagnosis TMJDS.Although Schüllar's projection is the most useful method andprovide informations of osseous,eondyle-fossa relationship and spaces of TMJ.The centric X-ray of the Schüllar'sprojection is not parallel to the long-axis of condyle.The aim of the study is to evaluate the two techniques withone case.The results have been proved arthrographically at last and analysised with statistics.According to 52 cases(104 TMJ)rediographically studies,there are 69 anterior spaces become wide in 69 ra-diograms of modified Schullar's projection and 21 radiograms in common Schüllar's projection,respectively.Statistics results reveal:anterior space x^2=45.658,(P<0.01);upper space x^2=6.744,(P<0.05);posteriorspace x^2=16.72,(P<0.01).It is obvious that the two technique have significant difference between the widen ofanterior spaces.The resnlts demonstrate that the corrected Schüllar's projection is better than the common Schüllar's projec-tion when use anterior space as a predictor to evaluate the disk displacement.
