摘  要:应用脉冲式电子耳蜗植入术治疗耳聋18例,取得良好疗效。该仪器的基本构成如下:1.录声微型话筒。2.脉冲模拟式(脉冲式)语言编辑器按脉冲模拟形式分析信息。3.透皮发射线圈。4.植入耳后颅骨的接收线圈。5.植入耳蜗鼓阶内的电极。选择对象为术前戴强放助听器不能识别语言;耳蜗电图表明耳蜗已失功能并无严重骨化。采用后鼓室开放术,精磨面神经管,在面神经隐窝圆窗龛前0.1~0.2mm开小窗达到鼓阶。术后详尽的听力学试验表明平均听阈为55dB(SPL),不适阈高达100~120dB(SPL)。环境声识别开放式为10~40%,限定式为40~80%。本组手术未发生并发症。Cochlear implantation of pulsive-analogue system has been performed in 18 cases of deafness since 1984. The cochlear implant is composed of a microphone, a pulsiveanalogue speech processor, a transmitter coil, a receiver coil implanted in bone and an electrode placed within the cochlea inside the scala tympani. Before operation, all candidates were unable to gain any speech recognition with powerful hearing aid and electrocochleography showed no useful cochlear function, There was no complication of implantation. Extensive audiometric testing showed that the postoperative hearing threshold was 55dB (SPL) on average. The discomfort threshold was higher than 100-120dB (SPL). The recognition soores of open-set of environmental sound improved to 10-40% with increasing scores (40-80%) in its closed-set.Our clinical experience proved that pulsive-analogue cochlear implant is a promising implant.
