牛心包生物瓣膜功能失调原因探讨

摘　　要：本文报道牛心包瓣膜替换术297例,长期生存253例,应用283只牛心包瓣。其中有16例瓣膜失败再次施行瓣膜替换术。症状有心悸、气急、易疲劳和下肢浮肿。体征有高调收缩期吹风样杂音,或舒张期吹风样杂音。超声心动图有人工瓣膜瓣叶在瓣口来回飘动。瓣膜损坏原因主要是机械疲劳引起支架顶端缝线撕裂。再沿瓣叶缝线至瓣窦底部。其次是瓣膜感染。手术结果存活11例,死亡6例。From April 1978 to Oct 1987, bovine pericardial valves were used in 297 cases of heart valve replacement. Among the 253 patients discharged from the hospital, 16 cases needed reoperation because of valve failure. All had palpitation, dyspnea, fatigue and lower extremity edema. The diagnosis of valve failure was confirmed by abnormal murmurs and echocardiography.The main features of failed valves in 11 patients were tear of leaflets at the commissure suture line near the tip of the strut. In another 5 patients there were evidences of chronic infection of bovine valves. The disruption of the leaflet was attributed to mechanical fatigue of the pericardial tissue.Reoperation for replacement of the dysfunctioning valve resulted in 11 recoveries and 5 deaths.
