三种生物瓣长期疗效的比较

摘　　要：本文报道1977年12月至1984年12月期中应用生物瓣(牛心包瓣280只、猪瓣181只、硬脑膜瓣32只)的远期疗效。术后生存出院的337例已随访3～10年,总随访期1869病人-年。随访中18例发生细菌性心内膜炎,发生率为0.96％/病人-年(牛心包瓣为1.1％/病人一年,猪瓣0.63％/病人-年,硬脑膜瓣0.55％/病人-年)。16例发生血栓栓塞并发症,发生率为0.86％/病人-年(牛心包瓣为1.01％/病人-年,猪瓣为0.63％/病人-年)。49例的51只生物瓣膜损破,损破率为2.62％病人-年,牛心包瓣的损破率为3.35％/病人-年,猪瓣为1.52％/病人-年,硬脑膜瓣2.2％/病人一年。牛心包瓣和猪瓣的损破率具有甚为显著的统计学差异(P<0.01)。牛心包瓣虽然血流动力学较好,但远期疗效远不及猪瓣。From Dec 1977 to Dec 1984, 493 bioprosthetic valves (homologous dura mater valves 32, porcine aortic valves 181 and bovine pericardial valves 280)had been used for heart valve replacement in 407 patients. 92% of these patients had rheumatic heart disease. There were 70 hospital deaths. The survived patients have been followed for 3-10 years. The incidences of bacterial endocarditis were 1.1%/patient-year for bovine valves 0.63%/patient-year for porcine valves and 0.55%/patient-year for dura mater valves The incidences of thromboembolic episodes were 1.05%/patient-year for bovine valves and 0.63%/patient-year for porcine valves. The incidences of valve failure were 3.35%/patient-year for bovine valves, 1.52%/patient-year for porcine valves and 2.2%/patientyear for dura mater valves. The difference between bovine and porcine valves was significant (P<0.01). Even dura mater valves seemed better than bovine valves in long-term results.
