摘  要:采用经椎弓根螺钉及撑开棍与压缩棍方式治疗脊柱侧凸15例,术前侧凸度数Cobb37℃110°,平均69.1°。椎弓根固定范围7~16节,平均12.3节;最高固定节段为T。手术后侧凸度数Cobb9°~30°,平均12.8°。侧凸矫正率53.3%~85.7%,平均71.9%。经椎弓相撑开(或加压缩)矫正率高,固定可靠,克服了目前常用方法的缺点;突破了胸-9以上不能用椎弓根钉的限制,是一种较理想的脊柱矫形内固定方法。The posterior spinal intrumentation for the treatment of scoliosis was widely and successful used for many decades.These system ,however,is not without its disadvantages,such as hook dislodged,rod breackage,fracture of purchase sites and dural,nerve root and spinal cord injury.and so on. From l988 to l993, an innovated transpedicular distraction- compression instrumentation system,which developed and reported previousely by the first author(J H ),was performed on l5 cases of scoliosis.Eight of them suffered from idiopathic scolicsis and seven from congenetal scoliosis Degrees of preoperative curvature were from 37 to 110 degrees of Cobb with an average of 67.4 degrees.Po- stoperative curvature degrees were 9 to 30 degrees with a mean of l2. 8 degrees.The average rate of correction was 71.9%. The number of segmental level was from 7 to l6 with an average of l2.3 se- gments,The highest one was T-1.The average period of follow- up obscrvation was 26 months The results were satisfactory. The transpedicular distraction- compresstion instrumentation system provides a powerfull three-deme- ntional correction of scoliosis with a rigid fixation,higher corrective rate and less complications.
