胃粘膜癌前病变中医治疗的临床研究

摘　　要：胃粘膜癌前病变(PCL)可由多种癌前疾病演变而来。本文报道1987～1991年中医辨证论治加化淤、抗癌中药治疗。由慢性萎缩性胃炎(CAG)引起的PCL44例。胃镜复查31例,疗效比较满意,现报告如下: 1 对象和方法 1、1 对象 本组男24例,女7例,年龄32～66岁,平均44.8岁。PCL的诊断标准:依照我国1990年出版的《中国常见恶性肿瘤诊治规范》(第四册胃癌)中对胃癌的癌前病变的规定,和WHO关于胃的癌前期变化的意见为根据,出现胃粘膜上皮异型增生(Dys)或肠腺化生(IM)者列入PCL,各型并分为轻、中、重三级,对一个患者活检标本上同时出现Dys和IM时,只计算Dys。 31例中PCL中,Dys型19例(轻度4例,中度14例,重度1例);IM型12例(轻度2例,中度6例,重度4例)。癌前疾病二级浅表性胃炎(CSG)1例,CAG30例,其中重度15例,中一重度4例,中度9例,轻度2例。Precancerous lesion of gastric Mucosa is derived from multiple precancerous lesions.This paper reports 44 cases of PCLGM caused by chronic atrophic gastritis (CAG). The treatment measures of PCLGM have been decided to adopt TCM differentiation, and resolving stagnancy; anti-cancer herbs, ''weikang' , a traditional Chinese compound herbs, and mixture of the above measures. 31 cases are examined by gastroscope,the result of which shows that effective rate is 83. 9 per cent,extinction 61.3 per cent ,and the resolution of the mid-abnormal hyperplasia 81.3 per cent and the resolution of the hyperplasia gravis, when using these measures, is difficult. Among three therapies;the most improvement of the PCLGM belongs to such types of diseases as cold-deficiency ,Qi-stagnancy and damp heat of the spleen and stomach. The shortest treatment duration is 3 months and generally speaking it needs half a year or one year. It has been turned out that TCM herbs and therapy can stop the development of the cancer,and suggested that the key points in the prevention and treatment of TCM therapy and herbs should be in the mid-abnormal hyperplasia and large intestine metaplasia. Surgery operation should be resolutely used to take away the stomach from mid-abnormal hyperplasia to gravis hyperplasia.
